Military Case Manager - Social Worker (Full Time)

As a Military Case Manager-Social Worker for Aurora Behavioral Healthcare San Diego, you’ll provide quality case management and psychotherapeutic services to adult psychiatric patients and their families. You will serve actively and constructively as a member of the multi-disciplinary treatment team, supporting the organization and program or unit philosophy of care; to ensure the safety and well-being of each patient.

*Pay Range: $30/hr. to $46/hr.

Responsibilities: Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members

• Provides group and family process-oriented therapies as well as conducts educational and other didactic groups for patients and family members using various professional treatment modalities
• Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient's aftercare need.
• Attends regular treatment team meetings to provide social work perspective
• Reports to community/state agencies any patient issues as required and represents facility in any legal proceedings

Skills:

• Excellent written and oral communications skills
• Skills in case management, time management, and crisis intervention common to acute psychotic as well as to non-violent crises intervention practice
• Knowledge of acute adult psychological disorders; advanced principals of abnormal psychology as specifically applied to adults; familiarly with follow-up resource services available to adults
• Skills in conducting marital and family group therapy
• Skills in maintaining information as highly confidential; ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation

Requirements

• MUST posses a MASTERS degree of SOCIAL WORK (MSW)
• Current BLS for Healthcare Providers
• Experience with Military Culture
• 1-year experience in an acute psychiatric healthcare setting preferred
