Position Summary: The Mental Health Specialistis responsible for providing approved MHR services to recipients assigned to them.
Qualifications:
Meets the following criteria:
1. Has a Master of Social Work degree or a Master of Arts degree in a mental health related field or a Master of Science degree in a mental health related field or a Master of Education degree in a mental health related field.
2. Has a minimum of 15 hours of graduate level course work and/or practicum experience in applied intervention strategies/methods designed to address behavioral and/or emotional and/or mental problems. These hours may be obtained as a part of or in addition to the Master's degree.
Major Task List:
1. Provides community support work counseling to recipients, as assigned.
2. Accepts assignment as Primary Community Support Worker and performs responsibly in this role.
3. May provide Group Psychosocial Skills Training to recipients, as assigned.
4. Participates in staff training assignments, as requested.
5. Submit accurate and complete weekly counseling notes and service logs.
6. Participates in the agency's Performance Improvement initiative.
7. Performs other non-direct service activities as requested by CEO, COO, or Program Director.
8. Accepts supervision from LMHP and agency management team.
9. Attend/conduct weekly staffing.
10. Communicates effectively and in a timely manner with other clinical/administrative staff, as needed for client related issues.
11. Functions well in a crisis, using good judgment and keeping appropriate staff informed.
12. Assess, review or monitor a recipient's status in relation to rehabilitation.
13. Identify, report and maintain weekly appointments for community resources.
14. Effectively plans work.
15. Works independently, as needed.

PERFORMS close observation, fall protocol and one to one protocol as directed. 2. IMPLEMENTS PATIENT CARE ACTIVITIES under the direction and supervision of licensed nursing personnel. PERFORMANCE STANDARDS A. PERFORMS vital signs, weights, ADL, nutritional monitoring, and communicates findings to the nurse. B. ASSISTS patients with laundry, meals, bathroom functions, and maintaining of belongings as needed. C. ASSISTS at risk/physically challenged patients with mobility needs as needed. D. INDIVIDUALIZES all patient care activities to the age specific/disability/ cultural sensitivity needs of the patient. 3. COMMUNICATES and DOCUMENTS patient observation and date effectively. PERFORMANCE STANDARDS A. DOCUMENTS patient's departure from unit (i.e., for passes, transport to doctor's appointments, community meetings) with 100% accuracy and within defined time lines. B. DOCUMENTS significant occurrences, observed behavior, vital signs, nutrition intake and patient observation findings on assigned patients and as requested by the registered nurse, with 100% accuracy and within defined time lines. C. COMMUNICATES outcome of assigned tasks to nursing staff in a timely manner. D. COMMUNICATES follow-up on patient care responsibilities; incomplete assignments, ongoing activities, and current patient needs to relevant staff to assure continuity of care. 4. DEMONSTRATES a positive attitude, professional boundaries, and accountability in all professional contacts. PERFORMANCE STANDARDS A. COMMUNICATES appropriately and effectively with patients in a manner sensitive to their age/disability and cultural needs. B. MAINTAINS knowledge of changes in policies and procedures applicable to position and patient care. C. MAINTAINS patient confidentiality, RESPECTS patient rights, and treats patients with dignity. D. DEMONSTRATES knowledge of patient neglect/abuse and appropriate reporting/ communicating of such. E. WORKS well with others, PROMOTES cooperation among staff members, contributes positively to setting to facilitate patient outcomes. 5. PERFORMS all other duties and special projects as assigned.
