St. Vincent's Medical Center in Bridgeport has more than 3,200 employees. It includes a 473-bed community teaching hospital, a 76-bed inpatient psychiatric facility in Westport, a large multispecialty provider group, and special needs services for adults and children. St. Vincent's is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network, and is the system's second-largest hospital.

JOB SUMMARY

The COI Program Manager is responsible for the development, coordination and oversight of the care continuum program for total joint replacement and spine patients at St. Vincent's Medical Center's Connecticut Orthopaedic Institute. This position leads the multidisciplinary care team and oversees the daily functions of patient navigators.

JOB RESPONSIBILITIES

  • Oversees daily operations and program development for the SVMC Connecticut Orthopaedic Institute (COI), including clinical pathway design, patient experience, budgeting, community outreach, and strategic planning. Manages team of Joint Replacement and/or Spine Program Patient Navigators to meet patient quality and safety goals.

  • Leads monthly Orthopaedic Committee meetings and data reporting sessions; directs multidisciplinary team in identifying and implementing program enhancements to achieve high quality and regulatory standards. Monitors and oversees relationships with post-acute providers to ensure quality in care transitions and patient outcomes. Leads planning, development, and implementation of joint replacement and orthopedic program goals and operations.

  • Cultivates and maintains physician relationships and serves as a liaison to surgeons' offices to promote standardization of care throughout the continuum.

  • Champions the HHC/SVMC COI orthopedic growth initiative to increase market share in the South region; promotes physician engagement and drives quality improvement initiatives while spearheading multiple projects- including Bone Health Program.

  • Drives continuous performance improvement within the organization while applying HHC mission and values. Analyzes joint/spine program data to identify opportunities for improvement and implement program changes as needed based on data results. Leads data analysis and submission to key insurer payers to achieve and maintain participation in national payer distinction programs.

  • Implements policies and procedures to ensure high quality of patient care, and maintains compliance with regulatory guidelines from external review entities, including The Joint Commission, Department of Public Health, and others. Coordinates and oversees the ongoing disease-specific certification process to continually improve care for the orthopedic/spine patient population.

  • Serves as the business liaison between surgeons, nursing management, staff and hospital administration regarding implementation and operational elements of the program. Coordinates the efforts between the surgeons, leadership, nursing management, and staff while tracking the progress of the program and identifying areas requiring attention including quality/outcome management, patient satisfaction, and cost-effective care.

  • Participates with hospital marketing department and local media to promote community awareness of the program. Promotes community awareness through seminars to highlight hospital and physician services.

  • Plans and implements ongoing formal and informal education programs to meet identified teaching needs of staff caring for joint/spine patients. Collaborates with team to develop in-service programs on new policies, procedures and changes in patient management.

Qualifications

Education:

  • Baccalaureate Degree in a health related field required.

  • Requires current CT licensure as one of the following: RN or PT

Preferred Education:

  • Master's Degree in health related or business field desirable.

Experience

  • 3 years in orthopedic clinical environment

Preferred Experience

  • Greater than 5 years in orthopedic clinical environment

  • Leading Joint Commission disease specific program surveys

Licensure, Certification, Registration

  • Registered Nurse or Physical Therapist with current Connecticut license.

Preferred Licensure, Certification, Registration

  • Certified in Orthopaedic Nursing

Knowledge, Skills and Ability Requirements:

  • Highly effective verbal and written communication skills.

  • Excellent interpersonal skills with strong ability to build collaborative relationships internally and externally.

  • Computer literate with experience using Microsoft programs.

  • Database management experience.

  • Ability to develop and mentor others.

  • Excellent program and project management skills.

  • Comprehensive knowledge of the field's concepts and principles.

  • Ability to manage and organize multiple priorities.
