Overview
This position works with the Rehab Care Manager team in a fast-paced, continuous learning, patient-centric environment to manage patients transitions between levels of care. Rehabilitation and skilled nursing knowledge is used to appropriately determine level of care needs, working with the patient care team to create a rehabilitation plan. A warm, caring personality, with exceptional communication skills is necessary to quickly build relationships with referral sources, patients and their families. Works with the acute hospital to identify appropriate referrals to post-acute and outpatient programs. Meets with patients and their families to create a plan transition, educating in preparation for the next level of care and recommending outpatient and community services to promote optimal functional and medical patient outcomes and best use of health care resources. Supports entry into Younker Rehabilitation at IMMC, Transitional Care (skilled level of care) at ILH, Outpatient Therapy and a variety of other programs to assist the patient and family in a successful transition home.

Responsibilities
Clinical Excellence & Patient Care
Assess the individual's personal and medical history, current status, diagnosis, prognosis, treatment plan, knowledge base and rehabilitative potential to determine strengths, challenges, prognosis, functional status, goals, and needs for specific services and resources.
Develops a plan that identifies short-and long-term goals, and integration of the community re- entry plan. Involves the individual, family, and interdisciplinary collaboration across the continuum.
Evaluates trends in patient outcomes related to use of resources and leads to decrease use of non-beneficial resources.
Reports any questions or concerns regarding compliance immediately to the attention of department or organization/hospital administrative staff.
Manages resources effectively to meet the needs of persons served while eliminating waste. Discusses payment benefits with patient and family. Applies knowledge of payment options for various levels of care, balancing individual patient needs and service requirements with UPHDM resources.
Manages personal and professional skills and knowledge, sharing expertise with others to enhance the overall skill level of the profession.
Demonstrates initiative to improve quality and customer service by striving to exceed customer expectations.

This position may be assigned a specific mission to address in an emergency situation to assist in disaster response and recovery as part of the UPHDM Hospital Emergency Incident Command System. Situations of a critical nature may require an individual to perform multiple tasks until additional support can be obtained.

Qualifications
Minimum Requirements
Preferred or Specialized
Education:
Graduate of accredited nursing program
BSN degree
Experience:
A minimum of 2 years of healthcare experience required with demonstrated professional and technical competence, including experience in the rehabilitation of chronically or catastrophically ill or injured individuals.
Experience in managing complex health, disability and financial resources with person served.
Outgoing healthcare professional with a demonstrated ability to consistently work with unfamiliar individuals in a variety of community settings with little or no supervision.
License(s)/Certification(s):
Current licensure in good standing to practice as a Registered Nurse in Iowa.
Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire.
Valid driver's license when driving any vehicle for work-related reasons.
Case Management, Care Coordination or other leadership approved specialty certification.
