The Family Wellness Coach works in partnership with children and families enrolled at the behavioral health services of CHI St. Joseph Children's Health. Through guidance and support, the Coach and family work to build the skills, resources and strategies to create opportunities for stronger, healthier, more engaging and fulfilling family relationships that allow every member of the family to reach their full potential. We believe that by improving the strength and capabilities of the family unit (however that is defined) we provide every child with the best opportunity to grow up happy and healthy.

Responsibilities

At CHI St. Joseph Children's Health the Family Wellness Coach:

  • Conducts family wellness assessments.

  • Provides focused interventions for clients and families interested or in need to enhance and foster the client and family's optimal functioning and coping both in the community and at home.

  • Assesses client needs, service availability and appropriateness of services.

  • Coordinates care with the psychiatric providers, therapists and case managers of the children's behavioral health department, as applicable.

  • Provides clients with self-management skills and educational information needed so they can be full participants in their identified goal plan;

  • Consults and coordinates with alternative agency services such as Early Intervention and Lancaster Intermediate Unit to establish continuity of care.

  • Documents the clients' progress and diagnostic information in the client's family wellness chart.

  • Develops and leads group level workshops and trainings surrounding family wellness topics.

Position Type: Regular Scheduled Hours per 2 week Pay Period: 80 Primary Location: PA > LANCASTER > ST JOSEPH CHILDREN'S HLTH

Qualifications

Education (including licenses, certifications):

  • Master's degree in Early Childhood Dvelopment, Family Systems, Behavioral Health, or Health Sciences Preferred (Social Work, Psychology, Nutrition, etc.)

Experience required and preferred:

  • Minimum of 3-years experience, working with children and families, preferred.

  • Bi-Lingual (Spanish) strongly preferred.

  • Knowledge of Evidence-based best practices and trauma informed services.

Skill Competencies:

  • Excellent communication skills - both written and verbal

  • Experience and ability to develop and deliver group level training workshops

  • Comfort working with computers

  • Ability to handle multiple tasks

  • Demonstrate a high comfort working with diverse populations and an understanding of the cultural and social barriers that can exist when accessing health or health-related care.

  • Promote a positive team-based approach to quality early education and care

  • Comfort with delivering in home and community based services.
