RADIAS Health is hiring a full-time Mental Health Practitioner to join the Whatever It Takes (WIT) Team. The WIT program specializes in supporting individuals with serious and persistent mental illness in returning to - and remaining in - community settings after extended stays at Anoka Metro Regional Treatment Center (AMRTC) or St. Peter Regional Treatment Center (FMH).

 This person must have excellent clinical and problem solving skills, strong communication skills, and a good understanding of community resources in the metro area. Some duties include: coordination with community providers and providers at hospitals to facilitate client discharge, assistance in establishing additional supports/services, supporting the development, writing, implementation, evaluation and revision of treatment and crisis plans in collaboration with the client and the treatment team, and utilization of inventive approaches to assist a client so that they can remain in the community. The practitioner will coordinate services with other county or community agencies including, but not limited to, AMRTC, FMH, Mental Health Case Management, Adult Protection, Health Partners, and community hospitals.  

Hours: Monday- Friday 8 am- 4 pm or 9 am- 5 pm

The Mental Health Practitioner position is a hybrid model. Some work is done in the community with clients and other work can be done from home.

Duties & Responsibilities

  • Work with the larger treatment team to provide services for clients.  Support the development, writing, implementation, evaluation and revision of overall treatment goals and plans in collaboration with the client and the treatment team.
  • Develop an individualized crisis plan coordinated with community emergency support services.
  • Meet with clients to explain WIT services, assess client needs, establish and implement a care plan to address those needs and arrange for resources necessary for community transition. 
  • Coordinate services with other county or community agencies including but not limited to AMRTC, MSH, Mental Health Case Management, Adult Protection, and community hospitals.
  • Assist in the development of comprehensive assessments, individualized treatment plans, substance abuse treatment, and other tools necessary to assist clients in the community.
  • Assist clients with all aspects of psychiatric rehabilitation and all aspects of obtaining and maintaining housing.  Will assist and /or teach clients independent living skills such as cooking, shopping, cleaning, personal hygiene, transportation, and budgeting.   Will assist clients with making and keeping appointments and any other tasks that arerequired through the client’s recovery process.
  • Attend commitment and other court hearings as needed.  Complete required court reports if requested.
  • File reports with Vulnerable Adult, Child Protection, the Ombudsman’s Office, and other mandated/required reports when applicable. 
  • Actively and assertively engage and reach out to clients’ family members/significant others, including landlords, employers and other treatment team members after obtaining client permission.
  • Provide on-call crisis intervention as determined by the Program Director
  • Must be able to transport clients in the community.
  • Complete all paperwork, reports, and charting contemporaneously and in an organized manner.

Requirements

Candidate will have a Bachelor's degree in Psychology or Social Work plus 2,000 hours work experience providing services to adults diagnosed with a serious mental illness

-OR-

No college degree and 4,000 hours supervised work experience in the delivery of services to adults or children with mental illness.
