Job Title: Memory Disorder Clinic Coordinator LCSW

Job Summary

The MJH MIND Institute's Licensed Clinical Social Worker / Memory Disorder Clinic Coordinator is responsible for the direct social work services for the patients, participants, and caregivers of the MJH MIND Institute. Delivers a broad spectrum of social work services, such as supportive therapy, case management, education, counseling, crisis management, advocacy, and referrals, in a qualitative, empathetic, and caring manner.

Essential Job Functions

  • Delivers a broad spectrum of social work services directly to MIND Institute patients and caregivers, such as supportive therapy, case management, education, coaching/counseling, crisis management, advocacy, referrals, etc. as needed and appropriate, in a qualitative, empathetic, and caring manner.

  • Collaborates with MIND Institute clinical team members and other professionals to evaluate patient's condition and situation to assess for patient needs and intervenes appropriately.

  • Provides individual brief therapeutic coaching/counseling for MIND Institute patients and caregivers as needed.

  • Performs initial screening and intake for all potential patients of the MIND Institute / Memory Disorder Clinic.

  • Reviews intakes with Chief Medical Officer for new patient approval and provides appropriate referrals and provision of services. (Sources include via phone, email, walk-in, and online Enquire requests).

  • Facilitates and coordinates consults and referrals of MIND Institute patients and caregivers to appropriate community or social service agencies, programs or facilities, and acts as liaison for them with such organizations as needed. Ongoing research for new referral resources and programs for patients and caregivers.

  • Intervenes with MIND Institute patients/caregivers in crisis situations to assist as appropriate.

  • Provides social work services for SGI Research participants as needed.

  • Acts as Coordinator for Non-Pharmaceutical Caregiver Studies/Pilot Programs (Care Heroes [completed] and Meaningful MINDs Expanded Program, both with FIU).

  • For Meaningful MINDs Brain Fitness Program, monitors client performance and adjusts activities and performance levels as appropriate. Communicates with HNP Technical Staff for any technical problems or issues during ZOOM sessions and for clients working at home.

  • As the Memory Disorder Clinic (MDC) Coordinator, participates in various external committees and group organizations (DOEA, DCCI, ADAC, Alzheimer's Association, Lead Agencies, County/City Govt. Agencies, First Responders, etc.) to advance the goals of the DOEA, MDCs and other related agencies for the betterment of patient health and well-being.

  • Facilitates the MIND Institute's caregiver support group, which is also open to the general community via promotion from the Alzheimer's Association for community outreach and education efforts.

  • Emails educational info and resources discussed during group sessions to caregivers and responds to individual questions. Ongoing update of Caregiver Support Group Email List.

  • Assists with community outreach and marketing initiatives for the MIND Institute.

  • Facilitates the MIND Institute's annual survey process and results report via Survey Monkey according to MDC/DOEA requirements.

  • Serves as Coordinator for all Internship Programs for MIND Institute.

  • Assists Chief Medical Officer of the MIND Institute with EmpathiCare and related trainings/programs as needed.

  • For Meaningful MINDs Brain Fitness Program, performs all private pay credit card billing, and sends daily report and receipts to Associate Practice Manager for Superbill process. Sends receipts to clients/caregivers. Assists clients/caregivers to correct any Program billing errors.

  • Attends and completes all team meetings, trainings, and organizational training as needed and appropriate.

  • Encourages effective relationships among teammates geared to the building and maintenance of a cohesive team.

  • Performs other job-related duties as assigned.

Job Requirements

Education:

  • Master's degree in clinical social work required.

Experience:

  • Three years of related experience

Licenses/Certifications:

  • Licensed Clinical Social Worker; Active Florida License required

Abilities Required

  • Excellent assessment and counseling skills

  • Thorough knowledge of psychosocial dynamics of individual and family

  • Excellent communication and interpersonal skills

  • Knowledge of geriatrics/gerontology preferred

  • Ability to work closely and well with other disciplines

  • Must be computer literate

  • No routine exposure to blood/body fluids/tissues but may occasionally require performing unplanned activities which may involve exposure to blood/body fluids/tissues
