Summary

As an LPN Charge Nurse in Hoosier Villages Memory Care, you are responsible for the supervision of all nursing activities during each shift, and supervising QMAs and CNAs providing direct care to residents, within scope of practice. This is a Monday through Friday position, with 8-hour shifts. We require our Charge Nurses to participate in the on-call schedule. You will care for residents by performing the following duties:

Duties and Responsibilities

Nursing:

  • Responsible for admission, transfer, and discharge of residents to and from Hoosier Village.

  • Responsible for administering medications and treatment, and maintaining proper charting for the unit, in accordance with federal, state, and local standards, regulations, and departmental policies and procedures.

  • Receives, transcribes, and implements physicians' written or verbal orders.

  • Assists physicians in making rounds, performs nursing procedures, and discusses specific resident's progress.

  • Receives and gives accurate, timely verbal and written reports on residents' condition upon shift change. Confers with the director of nursing to provide continuity of care and relates information to other nursing shifts to assure follow-up of physician's instructions.

  • Makes routine rounds of assigned residents and modifies standard nursing procedures for care in specific areas.

  • Responsible for assisting in maintaining the Hoosier Village and BHI's facility and nursing care philosophies, through quality nursing.

  • Responsible for maintaining a high standard of resident care.

  • Responsible for all mandated Hoosier Village, Federal and State annual hours of in-service education.

  • Promote and respect the dignity and well-being of each resident and their family and staff.

Supervisory:

  • Oversees nursing personnel in the memory care building daily. Communicate staffing changes with the nursing scheduler.

  • Work with the Director of Memory Care for any resident and family concerns in Hickory.

  • Collaborates with DON if clinical issues are involved.

  • Assist Director of Memory Care with evaluations of nursing staff working in Memory Care.

  • Participates with DON/Director of Memory Care in situations of disciplinary actions.

  • Oversees pharmacy consultant recommendations, medication cart audits.

  • Provides oversight for Infection Control Practices on the units and provides real time education when opportunities present and/or communicate needs for additional education to DON.

  • Consults with DON/Scheduler to ensure appropriate use of staffing/evaluation of needs.

Qualifications

  • Active LPN license in the state of Indiana

  • Two years of experience in a long-term care setting preferred
