At Sagewood at Daybreak, the Memory Care Coordinator passionately promotes a healthy, activity-enriched lifestyle within the Memory Care community. Directs, coordinates and supervises services and programs provided by the care staff for the residents within Memory Care, ensuring exemplary service delivered with integrity, dignity and compassion. The Memory Cary Coordinator is responsible for the delivery of quality service by encouraging independence and ensuring each resident's physical, social, cultural, and intellectual well-being.Essential Job Functions: * Monitor residents and evaluate residents regularly and assess levels of care whenever there is a change in condition and/or to determine whether a resident is no longer appropriate for the setting, notifying Assistant Living Director as required. * Assist residents with simple requests as needed or find appropriate associate to assist * Learn resident preferences for opportunities to provide exemplary customer service, exceed resident expectations and enhance resident satisfaction. * Effectively supervise departmental operations on a day-to-day basis under the guidance of the department director or Executive Director. * Responsible for overall administration and management of the Alzheimer's residential area. * Coordinate with the Wellness team and other department heads to provide programs that meet the emotional, intellectual, social, physical, spiritual and vocational needs of the residents. * Actively support and facilitate wellness programs that meet the needs of each resident. * Respond in a tactful and timely manner to all concerns placed by residents, family members and/or staff. * Supervise medication programs. * Interface with physicians and other care providers. * Monitor all physician orders and ensure they have been followed. * Ensure familiarity with medical terminology necessary in the delivery of care services to residents. * Maintain contact with outside health care services pertaining to the resident's needs (i.e., hospital, outpatient service, home health, therapies, etc.). * Observe, report and document residents' services and requirements at each shift and notify Assisted Living Director of any changes in a resident's condition or needs. * Oversee medication control and coordinate with physicians for optimum health care of residents. * Handle emergency situations responsibly and calmly until aid arrives. * Conduct and/or assist with regular staff meetings and daily stand-ups. * Plan, prepare and monitor daily work schedules to meet labor targets. * Review payroll documentation to minimize missed punches, monitor overtime, and manage labor costs. * Plan and prepare assignment sheets for resident assistants. * Maintain resident confidentiality and privacy. * Perform other related duties as required.Qualifications and
