We are currently in need of a Memory Care Activities Assistant to engage residents in activities.

  • Develop and promote programs and activities for residents, family members, and guests to remain active and engaged

  • Create flyers and newsletters encouraging all to embrace our culture of vibrant living!

  • Incorporate SPICE (Spiritual, Physical, Intellectual, Cultural, and Emotional wellness) into the daily lives of residents

In order to qualify, you must have experience in a customer service oriented role and the demonstrated ability to quickly connect with diverse individuals. Advanced computer skills are required with the ability to engage and motivate others. Previous experience with dementia or Alzheimer's residents and demonstrated interest in working with seniors is preferred.

VIEW JOBS 4/17/2019 12:00:00 AM 2019-07-16T00:00 Job Description POSITION SUMMARY The Zone Coordinator works directly with the ACC and Zone Leaders and the Clinical Care Team on a daily basis. This position is responsible for supporting the administrative work of the clinical care teams serving Aetna Community Care members in a new innovative community based care management program. The CMA reports directly to the Zone leader and is an integral member of the inter disciplinary care team and they role is key to making sure the care teams meet program goals. Fundamental Components: * Carries out and coordinates administrative duties as assigned, which include assigning Cases and or tasks to the Primary Care team members, documentation of team meeting notes, handling logistics for scheduling and coordinating meetings, assist zone manager with time and attendance, new hire onboarding and other administrative functions. * Works efficiently, independently, task lists, and reports, meeting deliverables and deadlines. Generates required reports, spreadsheets and documents as requested to meet program outcomes and deliverables * Acts as a liaison and coordinates workflow and communication for the care teams, as well as, with internal and external entities e.g. community based programs, vendors and program partners. * Effectively seeks out and uses approved and available resources to problem solve and meet deliverables. Knowledge of Care Management Software system and the ability to generate reports for meetings and program planning purposes * Adheres to care management, privacy and confidentiality, and quality management processes in compliance with regulatory, accreditation guidelines, company policies and procedures. Other responsibilities as assigned. BACKGROUND/EXPERIENCE desired: 5 years experience in an administrative support role required; experience in healthcare, care management and/or managed care desirable. Sound proficiency in Microsoft office; experience in working on multiple systems platforms preferred. EDUCATION The highest level of education desired for candidates in this position is a High School diploma, G.E.D. or equivalent experience.
