Position Summary

The Medication Reconciliation Pharmacy Supervisor shall practice the profession of pharmacy with the utmost code of ethics demonstrating the organization's mission and values. The Medication Reconciliation Pharmacy Supervisor provides the administrative coordination and process oversight of the Advanced Clinical Pharmacist

  • Medication Reconciliation and Medication History Technicians. The supervisor is responsible for day-to-day operations for home medication list and medication reconciliation program. This role is responsible for assuring the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which their team serves. The pharmacist supervisor uses advanced knowledge, skills, and professional judgment in collaboration with other healthcare professionals through the transitions of care process. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the care process. The pharmacy supervisor will work directly with the certified pharmacy technician to review home medication lists to provide a streamlined medication reconciliation transition into the hospital. Cost-effective utilization of medications will be reviewed and drafted as part of the inpatient admission medication list and discharge medication list by the pharmacist. As needed, the pharmacist will provide recommendations and/or literature to prescribers supporting the use of formulary agent, as well as, education to clinicians and patients regarding medication management. The pharmacist supervisor will be an active member of one of the following hospital committees: P&T Committee or Medication Safety. This position is responsible for creating a patient and customer focused care delivery model. The supervisor will be responsible for attracting and developing a team of highly capable, committed staff through recruiting and interviewing candidates that have a passion for patients and customer centered care. The supervisor is responsible for maintaining benchmark accountability metrics that equate to the success of the program.

Qualifications

EDUCATION & EXPERIENCE:

  • B.S. or PharmD Degree from an accredited College of Pharmacy, Required

  • Minimum of 1 year as a Pharmacy Clinical/Operations Specialist, Required

  • Graduate of an accredited residency program, Preferred

  • 3 years' experience of hospital pharmacy required

  • OR

  • Hospital Pharmacy Management or advanced degree (e.g., MBA, MHA, MS), Preferred

  • Specialty board certification preferred, Preferred

  • Experience performing basic mathematical calculations, balance and reconciliation of figures

  • Experience working in Microsoft Office and facility's EHR (e.g., Meditech, EPIC, Cerner)

LICENSES, CERTIFICATIONS, & TRAINING:

  • Maintians current pharmacist licensure, Required

  • (PGY1) Pharmacy Practice Residency, Preferred

  • (PGY2) Specialized Residency in area of specialty, Preferred

  • National Provider Identifier (NPI)

The Medication Nurse is responsible for administering injections, suppositories, enemas, and medications associated with wound care in addition to medications usually assigned to a Medication Technician. * Perform nursing duties associated with the Wellness Nurse role as assigned. * Coordinate medication services with resident, pharmacy and physician. * Order and track all medication changes, refills and deliveries with pharmacies. * Prepare medication for distribution to residents as per company policy. * Assist residents with taking their prescribed medications. * Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal and notify supervisor. * Maintain accurate, complete and confidential resident medication and care records according to company policy. Ensure all medication documentation is current and correct, including medication administration forms, physicians orders and dosage changes. * Keep medicine rooms and carts orderly, clean and appropriately stocked. Maintain an inventory of all department supplies and re-order as needed. * Control medication room/cart access at all times. * Monitor Psychotropic medication use is congruent with physicians orders and ensure resident behaviors warrant the use of medication. * Participate in and complete narcotic count at beginning and end of each shift. Report any and all discrepancies to supervisor immediately. * Communicate with physicians, family members and vendors, as needed. * Understand the meaning and usage of common medical abbreviations, symbols and terms relative to the administration of medications as used by the company, pharmacists and physicians. Qualifications: * Must be a Licensed Vocational Nurse or Registered Nurse holding a valid California nursing license in good standing * Prefer one (1) year experience assisting with medications at a Residential Care Facility for the Elderly (RCFE) * Knowledge of and/or ability to learn the theory and practice of assisted living and dementia care * Able to work with seniors and patiently interact with cognitively impaired individuals * Able to be flexible, adapt and respond to change, make decisions in stressful situations and prioritize tasks and projects. * Able to process information and apply common sense understanding to follow and carry out written or oral instructions. * Able to analyze, solve and respond to problems or concerns. * Able to count and perform moderately complex math problems * Must pass a Criminal Background check and Health Screening tests, including COVID-19 Polymerase Chain Reaction (PCR) test within 72 hours of start date.
