Overview

The Medication Management Compliance Specialist - provides oversight and direction for the ongoing management and regulatory compliance of medication management across the outpatient clinics for Dartmouth Health's Northern Region. The Specialist is accountable for all aspects of controlled and non-controlled medication management and will work proactively to manage, monitor, and optimize best practices for diversion monitoring and prevention, medication error and adverse drug event review and response, and all components of the medication management process, from procurement through administration and monitoring.

This is a fullt time bennefited positionn. Monday-Friday 8am-4:30pm with 3 days on site.

Responsibilities

Coordinates compliance and regulatory functions to ensure that departmental operational activities meet legal requirements and accreditation standards for quality and safety. Oversees the ongoing management and adherence to best practices for all medication management processes to include procurement, storing, ordering, preparing, administration, dispensing (as applicable), and monitoring.

Particular focus will include key ambulatory risk areas such as medication administration record documentation including 340B program compliance, high alert medications, look-alike/sound-alike medications, medication reconciliation, and diversion prevention. Oversees the ongoing management and adherence to best practices for controlled substance medication management across Dartmouth Health's Northern Region outpatient clinics. Ensures compliance with all DEA and chain of custody requirements in partnership with DH Central Supply Pharmacy.

In collaboration with the Drug Diversion Detection and Prevention Program and Dartmouth Health's clinic leaders, implements and oversees comprehensive surveillance and auditing programs to detect drug diversion. Reviews and responds to actual or potential adverse drug events, adverse drug reactions, and medication errors. Leads and facilitates improvement efforts when significant trends and variation are identified, reporting metrics and work summaries to the D-H Medication Safety Committee.

Monitors and communicates as appropriate actual or potential drug shortages. In conjunction with system pharmacy colleagues and nursing education, participate in and provide leadership in multidisciplinary education activities for controlled and non-controlled medication management. Facilitates medication management-related committees and joins other system wide medication management committees as appropriate.

Leads and/or contributes to the development and operationalization of policies, procedures, and job aids to ensure medication management best practices. Leads and/or participates in projects related to medication management and/or medication use. Partners with DH Specialty Pharmacy to lead and/or facilitate the optimization of specialty pharmacy integration in various outpatient clinics.

Assesses medication product and process compliance to identify operational risks and develops and implements risk reduction strategies. Routinely monitors state and federal regulatory sites for changes, providing updates to leadership/front line teams as appropriate. Partners with Quality and Safety, Risk Management, and clinical and administrative staff on an ongoing basis to optimize best practices.

Partners with Quality, the Drug Diversion Detection and Prevention Program, and System Pharmacy in all investigation processes of suspected drug diversion incidents. Partners with Quality and Safety on root cause analysis, apparent cause analysis, and occurrence report reviews to address and mitigate risk in other similar areas. Serves as a liaison to affiliates and to the greater community.

Qualifications

  • Graduate of an ACPE-accredited School of Pharmacy, Doctor of Pharmacy (Pharm D) degree preferred

  • 1 year of combined clinical pharmacy/specialty and/or residency experience required, previous leadership experience preferred

  • Must have demonstrated ability to facilitate change and work collaboratively with diverse, multidisciplinary teams.

Required Licensure/Certifications

  • Must have a current, unrestricted pharmacist licensed to practice in the state of NH.
