Description:
This pharmacy technician is responsible for conducting medication histories for patients admitted to the medical center through the Emergency Department and compiling the information in the electronic health record. This involves direct patient interaction as well as communicating with family members, physician offices, and retail pharmacies. The ideal candidate should have retail pharmacy experience and knowledge of brand and generic medications. The successful candidate should also possess good communication skills, including the ability to interact successfully with patients, family members, pharmacists, nurses, and all other colleagues. Ability to promote teamwork is essential.
The hours of this part time position are 2 x 12 hour shifts, every other weekend, from 7:00 pm - 7:30 am. PTCB pharmacy technician certification is required, though may be obtained after hire.
Has knowledge and can demonstrate proficiency on pharmacy computer system (Meditech) and Emergency Department computer system (Picis).
Complete communication of any problem or situation to pharmacist and to next shift.
Communication to appropriate supervisor as necessary
Understands proper departmental downtime procedures
Answers telephones and relays messages when appropriate.
Triages workload independently and appropriately throughout each shift
Works collaboratively with the pharmacists, physicians, registered nurses, and other medical staff
Has vital involvement in the medication use system by obtaining a best possible medication history for patients admitted through the Emergency Department, including prescription, over-the-counter, and herbal medications.
Communicate effectively and courteously with patients and health care providers, using interview skills, listening techniques, medication knowledge, and available resources to develop a complete and accurate electronic list of medications and allergies for each patient
Clarifies/confirms accuracy of medication history via multiple sources, including but not limited to patient/patient family or caretaker report, patient discharge summary reports, or contact with outpatient pharmacies, primary care physician offices, or other sources as necessary
Obtains accurate allergy information for each patient, including allergen, reaction, and severity
Documents obtained medication and allergy histories in hospital electronic ordering system (Meditech) accurately and timely
Develops and/or maintains intermediate to advanced knowledge of common medications, including primary uses of each
Activity is documented Demonstrates understanding of National Patient Safety Goals and related departmental precautions and policies
Assists in the training and orientation of new colleagues
Performs specialty functions, including but not limited to workload evaluation, position justification, continuing quality improvement or other assignments in support of hospital and pharmacy operations, as assigned
Reports activities to appropriate hospital committees (Medication Safety, etc) as assigned
Performs all other duties as assigned
Qualifications:
Pharmacy Technician (PTCB) certification within 3 months of hire or PA Registered Pharmacy Intern enrolled in Pharmacy school.
High School Graduate/Equivalent
Technical School/1 Year College and 2-4 years experience preferred
