The primary purpose of this position is to: provide business process management and medical administrative support/oversight to Patient Centered Medical Home (PCMH) clinical teams and their assigned beneficiaries. Provides supervision to subordinate personnel less than 25% of the time. Coordinates health services management functions.

Assists the Group Performance Manager with the development, implementation, and tracking of short-range and long range goals for the Patient Centered Medical Home team's business plans and objectives as it relates to the Military Treatment Facility (MTF's) goals and objectives.
Assists the MTF's Access Manager by overseeing the PCMH team's access plans and goals.
Serves as the principal PCMH Team office manager performing clerical, office automation support, and administrative duties to ensure operations run efficiently.
Oversees data quality management for PCMH clinical teams.
Exercises supervisory personnel management responsibilities. Advises and provides counsel to employees regarding policies, procedures, and directives of management.

Collegiate level coursework in health care administration/management, information technology, data management or business is strongly preferred.
Attendance at the AFMS Patient Centered Medical Home, AFMS Primary Care Optimization, and Group Practice Manager Course will be encouraged for the incumbent.
Basic Life Support certification must be obtained and maintained per AF guidance.
Medical Administration and Medical Terminology certifications desired.
QUALIFICATIONS: In order to qualify, you must meet the specialized experience requirements described in the Office of Personnel Management (OPM) Qualification Standards for General Schedule Positions, Clerical and Administrative Support Positions.
Specialized Experience: One year of specialized experience for GS-0679-07 level positions which includes monitoring Patient Centered Medical Home (PCMH) team's business plan performance through statistical analysis of workload, population demand, and PCMH team operations, and proposes solutions to optimize health care delivery. Participates in strategic planning for the PCMH team. Assesses and/or forecasts demand, measures/forecasts capacity, and provides oversight of ongoing access improvement initiatives. Monitors and assists team personnel in formulation of plans concerning access, demand, staffing, and provider templates/schedules. Provides support for Access to Care Standards by building and maintaining provider templates, modifying appointment schedules in the Electronic Health Care Records System (EHR), coordinating patient referrals for care, and completing end of day processing. This definition of specialized experience is typical of work performed at the GS-06 grade/level position or equivalent in the federal service.
FEDERAL TIME-IN-GRADE (TIG) REQUIREMENT FOR GENERAL SCHEDULE (GS) POSITIONS: Merit promotion applicants must meet applicable time-in-grade requirements to be considered eligible. One year at the GS-06 level is required to meet the time-in-grade requirements for the GS-07 level. TIG applies if you are in a current GS position or held a GS position within the previous 52 weeks.
KNOWLEDGE, SKILLS AND ABILITIES (KSAs): Your qualifications will be evaluated on the basis of your level of knowledge, skills, abilities and/or competencies in the following areas:
1.Knowledge of PCMH principles, practices, evidence-based guidelines, and procedures necessary to provide and document effective patient care.
2.Knowledge of standardized processes and procedures for evaluating medical support operations business practices.
3.Knowledge and use of medical terminology to accurately prepare and edit written correspondence and reports IAW Medical standards.
4.Skill in basic data gathering methods to collect factual data and use basic analytical techniques to review the data.
5.Ability to utilize health information technology and computer systems in order to coordinate support for medical staff; and communicate effectively, both orally and in writing, using tact and diplomacy.
6.Ability to plan, organize, and direct the functions and staff of a small sized organization.
PART-TIME OR UNPAID EXPERIENCE: Credit will be given for appropriate unpaid and or part-time work. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week.
