Role Description

Oak Street Health takes a team-based approach to providing outstanding patient care. Our Care Teams build individual relationships across a panel of 500-800 patients. The Care Team is responsible for delivering excellent, high-touch, primary care, and coordinating the care of our patients throughout the healthcare delivery system.

Medical Social Workers are an important part of our Care Teams. High levels of flexibility, energy, attention to detail, and problem solving are required to be successful. You will be expected to build relationships with Oak Street Health patients, manage patient's care plans, perform proactive phone and in-person outreach to our patients based on their care needs, support Care Team decision making, participate in weekly care team meetings, and generally coordinate all clinical and auxiliary services needed to provide a high quality health care experience from Oak Street Health.

What we're looking for

We're looking for motivated, experienced social workers with:

  • A problem-solving orientation and a flexible and positive attitude

  • Prior independent care management or case coordination experience

  • Experience conducting home visits and providing telephonic support

  • Enthusiasm for working with adults with disabilities, older adults, and adults with severe and persistent mental illness

  • Experience working on a multidisciplinary team preferred

  • Other duties as assigned

  • Masters in Social Work or Counseling required, LCSW strongly preferred

  • Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where necessary)

  • Proficient PC skills

  • US work authorization

Our model supports our social mission of improving the health and happiness of our patients and the community. Role Description The Lead Coordinator is an important part of Oak Street Health. The Coordinator will be responsible for assisting the SW Program Manager with educational and training opportunities for the staff of the SW Program. They will help build and maintain the first year MSW internship program in both established and expansion markets, train and orient new social workers in expansion markets, and at times, be a floating Lead Social Worker where there is a local need for social work leadership. The SW Program Lead Coordinator will work with other regional and corporate stakeholders to ensure initiatives and educational opportunities are met. Candidate needs at least 2 years of Medical Social Work Experience, preferably at Oak Street Health. Previous leadership experience highly desired. State minimum license preferred. Responsibilities * Facilitates the execution of corporate initiatives and pilots in markets * Facilitates training and educational operations * Specific areas of responsibility may include: * Provides and facilities educational training/shadowing for new hires (both initial and ongoing) * Assists with building and maintaining 1st year MSW internship programs * Seeking and developing continuing education opportunities for medical social workers * Develops and builds community partnerships in expansion regions * Creates partnerships with local Universities ( to obtain intern or ceu) * Oversees and may assist travel arrangements for training of new SW hires * Assist and conduct interviews (in person) in markets without SW Leads * Interviews Bi-lingual SW Applicants (where needed/applicable) * Partners with Medical Social Workers on establishing pilots programs for company and professional growth * Other duties as assigned This role reports to the Social Work Program Manager.
