This 378-bed multi-specialty, acute care facility is dedicated to the highest level of women's health services, including maternity and newborn care with Level I, II and III nurseries at the adjacent St. David's Women's Center of Texas.

The facility also features a 24-hour emergency department, the Texas Institute for Robotic Surgery, the Bariatric Center, heart and vascular center, neurology and neurosurgery, kidney transplant program, inpatient and outpatient surgery, and acute inpatient and outpatient rehabilitation. St. David's North Austin Medical Center is also home to St. David's Children's Hospital.

POSITON SUMMARY:

This position is responsible for the provision of medical social work services, financial counseling, mental health counseling (either by LCSW or under supervision of LCSW), and related assistance to pre-transplant candidates, transplant recipients and their family members. Provides support and advocacy services and linkage with community resources as needed. Works cooperatively and effectively as member of renal transplant team.

EDUCATION AND EXPERIENCE:

Required: Completion of a graduate level degree in social work from a program approved by the Council of Social Work Education; minimum two years experience in medical social work.

Preferred: Experience in financial case management in health care preferred.

Bilingual in English and Spanish.

LICENSES AND CERTIFICATES:

Required: Current licensure in Texas as a Licensed Master Social Worker (LMSW)

Preferred: Current licensure in Texas as a Licensed Clinical Social Worker (LCSW)

POSITION SUMMARY Provides psychosocial support to Stem Cell Transplant/BMT and Blood Cancer patients, caregivers, and families throughout the transplant continuum of care. The social worker is responsible for the management of continuous patient assessment, screening, and evaluation. They are responsible for providing emotional support, counseling, advocacy, and the coordination and planning of discharge and transitional care. PERFORMANCE EXPECTATIONS * Performs a comprehensive assessment of psychosocial needs of assigned patients; Involves patient, family/responsible/significant others, develops, implements, monitors * Optimize patient outcomes and quality of life throughout the blood cancer/BMT trajectory. * Maximizes patient and family psychosocial functioning and adaptation to diagnosis, comorbidities, and disease related outcomes. * Maintains and promotes professional standards and development by participating with in-services, committees, staff meetings, and performance improvement activities. * Assists in development of staff and team through educational guidance on professional boundaries, effective coping and communication strategies. * Functions as a part of a multidisciplinary team to bring patients successfully through the transplant process. * Participates in formal training in the management of patients receiving cellular therapy. * Complies with all policies, procedures, and regulatory standards. * Provides counseling support to help patient, caregivers, family members and survivors of transplant cope with the blood and marrow transplant process. POSITION QUALIFICATIONS * Master's Degree in Social Work with LCSW (or working towards it), with current state licensure * Minimum requirements for accrediting bodies, JCAHO, state and regulatory requirements related to service delivery in the facility * Three years Social Work experience; acute care case management experience preferred * Certification in Social Work or Case Management, preferred * Blood and Marrow Transplant background/experience preferred * Ability to establish and maintain collaborative and effective working relationships * Ability to communicate effectively in oral, written and electronic formats * Demonstrates analytical and critical thinking abilities with pro-active decision-making and negotiation skills * Demonstrates an ability to perform specific competencies as identified HCA Austin TX
