Position

Qualifications:

  • 1 year of prior social work experience preferably in Hospice, Long Term Care, Home Care or Acute Care
  • LMSW Required
  • Medicare guideline experience preferred

Job Responsibilities:

  • Assess and document patient/family social, emotional, financial and psychological needs as well as resources on the Comprehensive Assessment
  • Assist patient and/or family with establishing DNR, advance directives and/or DPOA or Healthcare proxy as requested
  • Assess and arrange patient placement such as Long Term Care, Assisted Living Facilities, GIP and as dictated by patient/family needs
  • Work in both Hospice and Palliative care Programs

Care Management social workers help patients, families/caregivers understand a particular illness; cope with emotions of diagnosis and treatments. The social worker provides counseling regarding decisions needing to be made as they impact family, employment, and finances. Assist in addressing the psychosocial issues related to the health condition. Works in active partnership with the RN care manager, the disease manager and other disciplines to develop and implement a well-coordinated personalized plan to address patient needs across the continuum. Principle Accountabilities: % Of Time Spent 1. Based on Nursing and Care Management Departmental Screening Processes, patients will be identified for appropriate Social Work referral as early as possible. 5% 2. In conjunction with the RN Care Manager and Disease Manager, a coordinated plan be implemented based on care needs identified from preadmission or admission to discharge assuring each patient's identified needs are addressed. Identifies appropriate education needs of patients utilizing in-house and community resources. Facilitates post-discharge arrangements. Oversees that they are carried out in a safe and appropriate manner. Communicates plans/changes to healthcare team members. 30% 3. Completes psychosocial assessment consistent with department guidelines. Able to define and address patient continuum of care needs. Requires knowledge of human development and behavior, of social, economic and cultural institutions and their interaction as part of the patient/family system. 15% 4. Helps patients and families understand the illness and treatment options, as well as consequences of various treatments or treatment refusal. 5% 5. Helps patients/families adjust to hospital admission/treatments; possible role changes; exploring emotional/social responses to illness and treatment. 5% 6. Provides brief, targeted therapeutic interventions for identified problems. Focus on the adaptive capacities of patients/families/caregivers with the goal of preventing maladaptive behavior and enhancing recovery. Appropriately assesses and intervenes in situations of abuse, neglect, domestic violence and sexual assault. 10% 7. Employs crisis Intervention 5% 8. Identifies underlying mental illness; providing or making referrals for individual, family and group psychotherapy. 5% 9. Arranging for resources/funds to finance medications, durable medical equipment, and other needed services. Maintains working knowledge base of community/ government resources and programs. Develops and maintains professional relationships with community resources. 5% 10. Provides Advance Directive Counseling to inpatients, outpatients and to the community. 5% 11. Maintains knowledge base through continuing education consistent with MO. licensure regulations and applies knowledge to current practice of social work. 5% 12. Performs other duties as assigned. 5% Total 100% Education Masters degree from accredited School of Social Work, Licensed Clinical Social Work status required. Experience Employee must be able to communicate in a professional manner orally, in writing and electronically. Employee must be able to listen and to understand and explain complex situations. Must possess group work skills and collaboration skills. The assessment, care and treatment provided by the social worker will be consistent with the specific age related needs of the patient. The social worker is competent to care for patient age groups including the neonates/infants, children, children, adolescents, adults and geriatric adult patient. Social workers address and understand the unique needs of diverse populations with whom they provide services. Working Conditions Employee is required to sit frequently and walk occasionally. Employee must lift/carry a light (1-20 lbs.) load occasionally. Employee may be required to lift over 20lbs. Employee is occasionally required to bend, squat, crawl, kneel, climb, lift waist to overhead. Employee is frequently required to reach forward and reach overhead. For repetitive action, employee is required to use both hands for simple grasping and pushing/pulling. Employee is occasionally exposed to electrical hazards and CRT terminal work. (Occasionally: 1-33%, Frequently: 34-66%, Continuously: 67-100%) The most significant of duties are included but this does not exclude occasional work assignments not mentioned.
