SUMMARY OF RESPONSIBILITY - Job Summary (general statement of purpose or objective of position) Assess social and emotional factors related to patient#s illness, medical and nursing requirements in the home. Develop and implement a plan for home social work services, including education of the patient, family members and care givers.

Coordinate with other members of the interdisciplinary team and community resources to provide an efficient integration of care. MINIMUM REQUIREMENTS - Knowledge, skills, abilities, license, registration, certification, education, and experience Master Degree from Accredited School. Current valid Arkansas Driver#s License.

Current Licensed Medical Social Work (LMSW). LCSW preferred. One year experience
