Job Details:

The Medical Social Worker is responsible for working in a team structure with the Outpatient Psychiatry staff, to effect overall care coordination. Assists families in resolving complex personal and environmental difficulties which interfere with effective social functioning and acquiring maximum benefit from psychiatric medical care. Assisting patients and their families in understanding and learning to cope with illness, disability, and or mental health diagnosis.

Participates as a professional team member in patient care planning along with the hospital and community agency personnel. Provides education and consultation to health care professionals and the community regarding the impact of psychosocial factors. Responsible for the delivery of social work services which are available to the patient, the patient's family, and other persons significant to the patient. Well organized, directed, and integrated within Psychiatry and other departments of the hospital and community. Facilitating skill building groups with outpatient therapists within the Psychiatry Department. This position will collaborate interdepartmentally with multiple divisions of the hospital.

Masters: Social Work (Required)

LSW Ohio

• Ohio Licensed Social Worker
