Performs secretarial and clerical functions in direct support of the Home Care / Hospice Supervisors by processing telephone communications and assisting in the day-to-day operations.

1.Regular, consistent, on-site, and timely attendance.

2.Processes telephone calls and messages promptly, accurately, courteously, and efficiently.

a. Obtains and documents complete and accurate information.

b. Ensures messages reach the appropriate individuals in a timely manner.

c. Treats all callers with courtesy and respect.

3.Process patient information.

a. Obtains and verifies patient demographic and financial information.

b. Enters all information into the billing system per established policies and procedures.

c. Enters all charges/codes into billing system accurately (where applicable by office).

4.Prepares documents for efficient office operations.

a. Completes forms and prepares letters necessary for continuing patient care.

5.Facilitates interpersonal communication on a routine basis.

a. Interacts with external customers in a courteous and respectful manner.

b. Interacts with co-workers and internal customers in a respectful and professional manner.

6.Supports financial objectives (where applicable by location within health system).

a. Makes every effort to collect payments at the time of visit.

b. Accurately completes cash sheets.

c. Delivers cash and cash sheets daily to appropriate place.

d. Assists in maintaining/decreasing organizational/departmental costs.

7.Facilitates patient access to care (where applicable by location within health system).

a. Schedules patients for appropriate intervals and provider based on protocols.

b. Assists patients to access specialty care and testing per physician order (as directed by facility).

c. Provides instructions and directions based on protocols and physician order.

8.Demonstrates accountability for professional development.

a. Maintains accountability for actions taken.

b. Effectively functions as a resource person.

c. Acts as a patient advocate.

d. Performs effectively in the role of preceptor/mentor to new hires and / or students.

e. Demonstrates the ability to solve problems and seeks assistance from immediate supervisor when appropriate.

1.Works closely with Supervisors on staffing and scheduling issues

a. Monitors notification of supervisors assigning Case Managers for new patients.

b. Consults with Supervisor if patients must be moved between staff members

2.Generates an admission chart for all new patients

a. Creates an admission chart for every new referral in a timely manner

b. Reconcile that each referral is entered into Horizon Homecare

c. Types a label with patient name, SOC, patient number , etc. and inserts referral

3.Completes final discharge process of patient chart

a. Verifies all required signatures are present

b. Chart contains all required documentation

4.Prints and mails various orders to physicians for signature

a. Prints all 485's and orders in a timely manner

b. Mails orders to appropriate physician via various modes of transmission

5.Filing of patient documentation

a. Sorts incoming mail by patient and admission

b. Files documentation in patient chart in a timely manner

c. Contacts staff for missing consent forms

6.Orders and maintains adequate inventory of patient care and office supplies

a. Orders additional supplies as quantities run low

b. Submits special orders for supplies that we do not stock

7.Maintains scheduling boards

a. Makes all scheduling changes and updates on scheduling boards

b. Schedule changes are communicated to staff that are affected by the change

c. Extracts referrals from Horizon Homecare and places on scheduling board

d. Updates scheduling board each Friday for the upcoming week

e. Check Palliative Care User daily for new referrals

f. Print daily visit schedules

8.Process telephone calls

a. Processes customer telephone calls in a positive, courteous and efficient manner

b. Routes calls and messages to appropriate personnel

c. Provides visit information to patients and caregivers

These are performed under the direction of the Nurse and/or the physician in preparation for the patient's examination and treatment. The incumbent carries out clerical activities, quality control activities, performance improvement initiatives, etc. in support of the physician practice as assigned. This position also serves as a resource for the clerical/clinical staff in the practice. Essential Job Functions * Deliver direct and indirect patient care in accordance with hospital/physician practices policies, procedures, and protocols. * Assist to collects patient healthcare data, including VS, weight, chief complaint and pain assessment as directed. * Complete patient care treatments and testing as ordered. * Comply with completeness, accuracy and timeliness of documentation. * Prepare patients for examinations, treatments, and procedures. * Communicate pertinent information of actual or potential problems to the physician or nurse. * Perform clerical functions in support of patient care and office functions. * Answer telephone calls, taking accurate messages and relaying them to the appropriate staff. * Obtain and verify accurate patient demographic and insurance information entering it into the billing system. * Accurately enter codes, based on diagnosis given, and enters codes and charges into billing system. * Complete forms and letters necessary for continued patient care. * Provide for patient safety in compliance with hospital and physician practice policies. * Label all patient specimens with identifiers per policy. * Adhere to infection control process, including standard precautions, to reduce the risk of acquiring or transmitting infections. * Assist in continuous survey readiness or inspections and surveys. * Perform all quality control, monitoring functions and reports out of range information immediately to supervisor. * Consistently practice National Patient Safety Goals. * Knowledge of and/or ability to schedule surgery, procedures, and testing for patients. * Exit counseling which includes education, scheduling of testing and providing patients with necessary information. * Prepare charts for clinic sessions and obtain vital signs and weight. * Perform control studies and completes log books according to policy. * Perform phlebotomy procedures, reads the test results, and documents accordingly. * Prepare the exam rooms and stocks supplies daily. * Provide pre and post-examination education and counseling. * Other duties as assigned. Required Qualifications * High School Diploma or GED. * Successful completion of Medical Assistant program or experience as a Medical Assistant for three (3) years continuous. * Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice. * Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience. * Knowledge of Microsoft Office Applications, including Outlook and Excel. Preferred Qualifications * Previous outpatient office experience. License, Certification & Clearances * Medical Assistant Certification preferred. * PA Act 153 Clearances (Act 34-PA Criminal Record Check from the PA State Police system, Act 33-PA Child Abuse History obtained through the Department of Public Welfare, Act 73-FBI Fingerprint screening). * Current Basic Life Support (BLS) approved curriculum of the America Heart Association (CPR and AED) program. Supervisory Responsibilities * This position has no direct supervisory responsibilities, but does serve as a coach and mentor for other positions in the department. Position Type/Expected Hours of Work * Incumbent will be scheduled based on operational need (rotate shifts, standby, on-call, etc.). * Travel may be expected locally between Excela Health locations.
