JOB SUMMARY The Medical Navigator provides safe and appropriate basic patient care and administrative duties as needed, in order to assist patients in achieving established outcomes by collecting data points (e.g. vital signs and urine specimens) and other information to be utilized in assessment. These activities contribute to meeting the health care, safety and comfort needs of clients, and are performed in accordance with all existing regulatory, state and company standards, policies and procedures. KEY RESPONSIBILITIES 1.Process and transition clients through admissions including completion of all required documentation. Obtain and observe urine drug screens, take client vitals, and report abnormal values/trends to staff nurse. 2.Model quality client/ customer services by: demonstrating a willingness to serve all customers at all times; maintain an awareness of patient needs and utilize the initiative to respond to those needs within skill level; utilizes tact, discretion, and compassion in dealing with other members of the team to ensure quality patient care. 3.Demonstrate willingness for self-improvement by collaborating with leadership to identify learning needs (reflective of the organization and of the individual) and seek instruction from appropriate resources. 4.Orient clients to the rules and regulations for clients at Rushford. 5.Provide basic counseling to help decrease the client anxiety and assure continuity of care. 6.Meet with client's family obtaining information related to client's history as it pertains to admission approval. 7.Ensure admission is complete by following checklists for accuracy Verifies all required documentation is complete and accurate in client record while displaying empathy, dignity and respect to the client and their families. 8.Coordinate with all admission related staff ensuring expediency of client admission including UR, Nursing, Admission Specialists and staff in related departments. 9.Participate in H3W workgroup as assigned. 10.Perform additional responsibilities and /or special projects as assigned by supervisor. 11.Adhere to Rushford policies and procedures. High School graduate or equivalent. Diploma from an accredited medical assistant program, Certified Nurse Assistant certification, or Medical Certification required One year of keyboarding and computer skills with working knowledge of Microsoft Office products Knowledge of drug & alcohol treatment or disease concept of addiction preferred. Knowledge, Skills & Abilities Demonstrated organizational and multi-tasking skills Good interpersonal/written communication skills Ability to perform duties with minimal direction Customer Service and detail oriented Maintain confidentiality and professionalism at all times Able to function as a member of a team Familiarity with Alcoholics (or Narcotics) Anonymous 12-Step Program.
