HPM manages 30 locations throughout Long Island, NYC including the 5 boroughs, Westchester and NJ. Monday - Friday: 9:00 am - 5:30 pm

Duties and Responsibilities:

  • Responsible for reviewing all charts prior to the doctor's appointment to ensure that all medical records are on file
  • Obtain records from outside sources for preview prior to the office visit
  • Prepare complete Medical Records for the Billing Department as requested.
  • Create chart folders (includes adhering of all labels and hole punching)
  • Pull charts daily for patients who are scheduled
  • Review charts to make sure all patient demographics information is complete and accurate
  • Prepare patient charts to include demographic information, previous x-rays, tests, etc.
  • Collect all necessary paperwork and authorizations needed for billing
  • Scan information to patient's electronic record
  • Schedule and confirm appointments
  • Other tasks as assigned.
  • Preferred bilingual in Spanish & English

Assists in patient care and administrative duties necessary todeliver quality patient care. Performs various duties as a member of the healthteam in the treatment and care of patient and their families and also assistswith tasks to support the unit. Participates and supports the health team inthe direct patient care under the supervision of an RN/LPN or MD. * Monitors patient flow inwaiting room and resolves patient flow issues related to registration andpost-visit processing * Provides set-up of examinationroom and instruments in accordance with proper techniques established byhospital and infection control policies. * Takes and monitors vitalsigns, including fetal heart rate if necessary * Performs phlebotomy,point of care testing, and other procedures as indicated by physician order,including but not limited to electrocardiogram and phototherapy * Assists with proceduresas necessary including but not limited to echocardiogram, sigmoidoscopy,proctoscopy, nasal scope, colposcopy, biopsy, LEEP, laser and cystoscopy * Sets up oxygen andsuction * Performs visionscreening * Inventories, orders orre-stocks medical supplies; monitors expiration dates on all medications andmedical supplies disposing appropriately as per institution policy * Maintains logging systemas required for refrigeration of medications, point of care testing,sterilization process, or others as needed * Performs routinemaintenance/cleaning of equipment and instruments; Notifies appropriatepersonnel regarding any malfunction in equipment * Maintains a safe,secure, and healthy work environment by following standards and procedures;complies with legal regulations * Complies withestablished personal protective equipment requirements necessary for protectionagainst exposure to blood and other potentially infections body fluids,chemical disinfectants, radiation and other hazardous substances * Acts rapidly and calmlyduring emergencies, initiates CPR and provides the necessary items called forby the team * Transports patients andequipment as needed * Performs any and allduties involving direct patient care as assigned by the RN/LPN within theframework of the New York State Nurse Practice Act and MSHS Policy andProcedures * Documents in medicalrecord / electronic medical record as required by hospital policy * Greets and registerspatients, prepares and processes appropriate forms, directs patients toappropriate areas and arranges transportation needs * Screens incoming calls,takes and responds to messages, as well as responds to inquiries regardingdepartment procedures, services and programs. * Schedules appointmentsand completes pre-registration via telephone including insurance verificationand full patient demographics collection over the telephone or in person usingcomputerized system(s) with accountability for the integrity of the data,including all patient demographics and insurance information * Fulfills clericalresponsibilities as assigned using the electronic medical record (EMR) systemor manually as necessary, to include: sending and receiving patient medicalrecords; obtaining lab/x-ray reports, discharge summaries, referralinformation, etc. * Performs routine cashcollections at time of service. * Completes forms andrequisitions as needed, managing charts to ensure information is completed andfiled appropriately. Assists in the creation and maintenance of medical charts. * Retrieves lab, pathologyand diagnostic testing, and operative reports and data from computer systems * Checks, verifies andupdates insurance information utilizing internal or electronic sources. * Obtains and documentsmanaged care referrals, required pre-certifications and authorizations asnecessary * Supports surgical orprocedure scheduling in conjunction with Surgical Coordinator or other similarrole(s) verifying times with patients; preparing charts, pre-admissions andconsent forms * Maintains a workingknowledge of New York State third party payer eligibility requirements forprivate and hospital billing * Utilizes and understandsbasic medical terminology; able to complete visit encounter forms and entersICD10-CM and CPT information into computerized systems as appropriate * Assists in training,orientation and education of entry level and or/float staff.
