About the Job

Position Summary:
Under the supervision of the Site Manager, performs patient triage and has patient care responsibilities in support of multiple health care providers. Performs clinical, technical and clerical duties to insure quality patient care is provided.

Essential Duties & Responsibilities including but not limited to:

  • Receive patients calls, assess/triage patients' medical concerns or needs , route calls to providers, and/or handle calls within scope of authority.

  • Take detailed messages for providers, make follow-up telephone calls to patients with information or providers instructions (i.e. normal and abnormal test results and test preparations.)

  • Give routine injections and immunizations, perform finger stick and venipuncture phlebotomy and dipstick urinalysis. Provide backup support to phlebotomists.

  • Take patient vitals, patient histories, and record in patients charts.

  • Provide patient guidelines and education regarding all aspects of related health care.

  • Prepare patients for procedures and assist providers with procedures in treatment room (i.e. sigmoidoscopies, pelvic examinations, wart removal, dressing changes and surgical procedures.)

  • Manage patient flow, escort patients to exam rooms, take and record patient vitals and patient histories.

  • Insure all necessary paperwork is signed by parent or guardian, when applicable.

  • Document all school and camp medical forms and return to proper order, when applicable.

  • Perform electrocardiograms, holter monitor, spirometries and process and prepare basic labs and routine tests.

  • Perform housekeeping, handling hazardous waste, and keep exam rooms, medication sample closet and crash cart fully stocked. Sterilize and maintain instruments using autoclave equipment. Order medical supplies as needed.

  • Keep medication sample closet organized and maintains locked status at all times. Replenishes medication samples and purges outdated medications.

  • Document and process encounter forms, laboratory requisitions, state supplied injections and all necessary paperwork.

  • Orders durable medical equipment as requested by Provider.

  • Call in prescriptions to pharmacy.

  • Incorporates Lahey Clinic Guiding Principles , Mission Statement and Goals into daily activities.

  • Complies with all Lahey Clinic Policies.

  • Complies with behavioral expectations of the department and Lahey Clinic.

  • Maintains courteous and effective interactions with colleagues and patients.

  • Demonstrates an understanding of the job description, performance expectations, and competency assessment.

  • Demonstrates a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards.

  • Participates in departmental and/or interdepartmental quality improvement activities.

  • Participates in and successfully completes Mandatory Education.

  • Performs all other duties as needed or directed to meet the needs of the department.

Minimum Qualifications:

Education: Graduate of an accredited Registered Nurse or Licensed Practical Nurse program.

Licensure, Certification, Registration: LPN or RN licensed to practice in state of Massachusetts. Must maintain BLS certification.

Skills, Knowledge & Abilities: . Excellent communication skills to effectively educate and assist patients and interface with providers and office staff. Ability to make sound decisions under pressure and respond to emergency situations. Demonstrated ability to handle confidential material. Must have organizational skills and ability to follow through with tasks. In the absence of any of these skills, demonstrates the ability to be thoroughly trained to meet organizational standards.

Experience: One to two years office nursing experience, with strong triage skills

Also, The RN Registered Nurse job coordinates total plan of care with other health care professionals involved in care, and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting. * Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team. * Makes the initial and|or comprehensive nursing evaluation visit, determines primary focus of care, develops the plan of care within State specific guidelines, and submits accurate, complete, and timely documentation, per policy. * Regularly evaluates and re-evaluates (as warranted by changes in condition but at least every 60 days) the patient's nursing needs. * Performs patient comprehensive assessments at designated time points and develops the appropriate POC, in collaboration with physician orders. * Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source. * Initiates, develops, implements and makes necessary revisions to the plan of care in collaboration with the physician and other health care professional's involved in care. * Makes referrals to other disciplines, as indicated by patient's assessed need. * Responds to outcome coordinator|coder and Patient Care Manager requests for clarification to OASIS assessments on the same day that the request for more information is sent. * Plots patient encounters for the episode and determines needed RN encounters based on patient's needs and regulations. * Instructs and supervises the patient's family|caregiver in the care of the patient and maintenance of a healthy environment forthe patient. * Actively participates in weekly case conferences. * Maintains a current and accurate patient medication profile. * After start of care (SOC) assessment, reports the status of the patient, assessed needs, and plan of care overview to the teamleader on same day (or by next business day if after hours). * Observes, records and reports to the physician and/or team leader the patient's signs and symptoms, response to treatmentand changes in the patient's condition, as appropriate. Ensures adequate Team Leader (TL) communication when physicianfollow-up is needed. * Communicates changes in visit assignments, dates of scheduled visits, and schedule changes to scheduler and Patient CareManger to ensure patient needs are met. * Communicates timely and effectively with agency personnel and ordering physician as required to process orders and OASISdata sets, schedule home visits, and deliver services to patient as ordered by physician and in accordance with applicable lawsand regulation. * Facilitates hand-off communication to RN and PCM who will cover patients in their absence, prior to scheduled days off. * Performs regular and supervisory visits according to the plan of care and submits complete visit notes within 24 hours ofcompletion visit. * Directly and/or indirectly supervises care provided by the home health aides and licensed practical vocational nurses, providesinstruction as appropriate, and assigns tasks according to State and federal regulations. * Participates in on-call rotation. * Adheres to and participates in the agency's Episode Management process. * Assists in the orientation of new agency personnel as assigned. * Completes LHC required learning courses, additional assignments per DON request, as well as any state specific requiredtraining per state regulation|practice act requirements. * Participates in the performance improvement plan and process to ensure positive patient outcomes. * All other duties as assigned.
