  • Knows, understands, and puts into practice general lab policies/procedures as well as department/section specific policies/procedures. Reviews all posted policy changes as required. Adheres to all regulatory guidelines.

  • Demonstrates the ability to assess a situation, consider the options, and formulate an appropriate course of action. Recognizes and performs duties in an independent manner with little or no supervision. Possess the ability to assume and carry out "tech-in-charge" duties, including the ability to direct the activities of staff or functions, when necessary.

  • Demonstrate initial and annual competency in all appropriate designated lab sections within set deadlines. Accurately performs proficiency testing when scheduled. Participates in continuing education activities.

  • Participates in all facility/corporate mandatory annual training within set deadlines.

  • Is cooperative in assisting the laboratory team in the completion of miscellaneous duties. This includes specimen receiving and putting away supplies when received in the department.

  • Effectively organizes and completes daily tasks in assigned area{s}.

  • Effectively troubleshoots/resolves instrument problems and documents corrective action in appropriate log; when necessary, performs instrumentation repairs. Effectively investigates/resolves specimen collection problems using Outstanding Specimen Report (OSR) and specimen tracking tool when appropriate.

  • Assures quality control is properly performed and logged, works within quality control limits and trends, and follows proper department procedure for corrective action.

  • Preventative maintenance and safety checks of equipment are performed and documented according to schedule.

  • Demonstrates the knowledge and skills to train new employees and/or students in general lab policies/procedures as well as department/section specific policies/procedures.

  • Demonstrates knowledge and skill necessary to communicate with, provide care for, and assess data appropriate to the age of the patient served.

  • Demonstrates knowledge and adheres to all policies relating to the JCAHO National Patient Safety Goals for the Laboratory, including, but not limited to critical call read back and patient identification.


Required - Associate Degree in Medical Laboratory Technology or related science


Required- Registry eligible, ASCP or equivalent. Successful completion of MLT registry within 6 months of eligibility or hire. Continuing education to maintain registry/licensure is required.

CPR Certification required for Free Standing Emergency Departments.

Preferred- MLT(ASCP) or equivalent. Registry/Licensure in a specialty area will be considered. Continuing education to maintain registry/licensure is required

Why work at Portsmouth Regional Hospital? Job Description - RN Case Manager POSITION SUMMARY: Provides Utilization Review and Case Management functions for the organization. Performs preadmission, concurrent and retrospective review for all payers. Completes assessments for the purpose of identifying discharge-planning needs. Provides discharge-planning services for select patient populations. Collects data to support performance improvement initiatives. SPECIFIC ELEMENTS & ESSENTIAL FUNCTIONS: 1. Applies rules of Severity of Illness/Intensity of Service/Discharge Screens (IS/SI/DS) consistently in evaluating medical necessity for governmental payers (Medicare/Medicaid/Champus) 2. Provides initial and ongoing assessments for patients/families having complex needs and/or medically complex diagnoses within 24 hours of admission or 1 business day; prioritizes needs ongoing 3. Accepts calls from physicians, MD office and ED, applying appropriateness criteria to all new admissions 4. Understands and applies critical thinking skills to reimbursement methodologies 5. Demonstrates awareness of current trends that influence health care practice and assures compliance with regulatory, governmental and organizational standards 6. Conducts all interactions with staff, patients and members of the public with dignity and respect, keeping with institutional corporate ethics. Reflects the hospital's values and possesses a sensitivity to the hospital's ethical and compliance principles in activities of daily work life (ADWL). 7. Identifies and intervenes to coordinate care on patients in relation to appropriateness of level of care, length of stay and resource utilization issues 8. Collaborates with the health care team and develops treatment goals and plans interventions that assist the patient and family to resolve concerns that may arise as a result of illness, engagement in risky behavior (i.e. substance abuse), limited finances and/or physical disabilities 9. Provides crisis and supportive counseling to enhance the patient's and families problem solving and coping skills. Facilitates the completion of advance directives, living wills, durable power of attorney and/or guardianship 10.Refers patients who require extended care post discharge to long term care, skilled nursing and/or acute rehab facilities; once bed is found, makes all arrangements for transfer 11. Collaborates with community agencies to provide services to assist with housing, financial, transportation, psychosocial, educational, home health, and home medical equipment needs of patients and families post discharge 12.Identifies, tracks and records potentially avoidable days (PAD's); intervenes to prevent or decrease PAD's as appropriate 13.Builds relationships with the hospital and external community mutually working towards the benefit of our patient's interest 14. Provides ongoing education for the other members of the health care team related to ethical, psychosocial and discharge planning issues 15. Educates patients and families to the patient's needs post discharge and informs them of alternatives available to meet those needs 16. Communicates daily with on-site reviewers to collaborate in treatment plan, goals and discharge plan 17. Initiates hospital issued notices of non-coverage (HINN's) when appropriate 18. Documents assessments, treatment goals, interventions, outcomes and discharge plan in the patient's medical record congruent with departmental policies 19. Initiates data collection regarding post-discharge referral patterns (triple space discharge log); Identifies and reports data to the Quality Management Department 20.Performs retrospective reviews for all payers 21.
