The Charles George VA Medical Center (CGVAMC), Nursing Service, is currently recruiting for one (1) Medical Instrument Technicians/RCIS (Catheterization) to work in our Cardiovascular Lab.
Work Schedule: 40 hours per week - Day tours with Call requirements: When on call, there is a mandatory 30-minute arrival expectation

The Medical Instrument Technician/RCIS (Med Tech) will perform, under a physician's direction, diagnostic tests, both invasive and noninvasive, of the pulmonary system (lung) and the cardiovascular system (heart and circulation). The incumbent operates, monitors, and collects data from instruments used in procedures such as cardiac catheterization, angiography, valvuloplasty, angioplasty, electrophysiology studies, cardiac pacing, or cardiac pacemaker or leadwire insertion. Medical Instrument Technicians/Cardiac Catherization Technicians function as full members of the Cardiac Catheterization Laboratory (CVL) Team.
Duties listed but are not limited to the following:
Scrub, circulate, operate equipment, etc. on the most complex examination and treatment procedures including procedures such as endocardial biopsy, radio frequency ablation procedures, electrophysiology studies and intra-aortic balloon pump therapy for very critical patients.
Prepare and administer cardiac medications as directed by the attending physician.
Assists the Supervisor/Nurse Manager with quality control and performance improvement activities.
Provides technical assistance to the Cardiologist during all procedures occurring in the Cath Lab. These procedures include, but are not limited to: LHC, RHC, and PTCA's, Stent deployment, IABP, atherectomies, pericardiocentisis, biopsies, temporary pacemakers, EP studies, permanent pacemaker and ICD placement.
Monitors the progression of procedures requiring independent judgment to provide specific instruments and/or catheters, as they are needed, in anticipation of request.
Provides accurate monitoring and recording of intravascular pressures and electrocardiographic tracings.
Operates digital radiographic computer systems, assists physician in procedure by performing scrub duties, injection of contrast, and/or panning during the procedure.
Trouble-shoots equipment failures and independently corrects the problem short of electrical involvement. Maintains calibration of instruments and calculation of all pressures, heart rates, and other recorded parameters.
Assists with transport with monitored patients from lab with RN post procedure when necessary.
Proficient at sheath removal using manual or alternative methods and use of groin site management devices (FemoStop, closure pads/devices). Must be able to identify issues and set up for IABP placement.

Certification: Licensure or Certification Registration by Cardiovascular Credentialing International (CCI) as a Registered Cardiovascular Invasive Specialist (RCIS) is highly desirable and 3 years of MIT experience in a Cardiac Cath Lab.
Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications are also desirable for employees at this level.
Documented ability to perform the duties listed at the full performance level (GS-8) Grade Determinations:
Experience: Must meet basic requirements and have at least one year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, abilities and other characteristics related to the duties of the positions to be filled. In addition, the candidate must demonstrate the following KSA's:
1.Knowledge of cardiac anatomy, physiology, and basic electrocardiography.
2.Knowledge of sterile techniques, catheterization laboratory supplies, and equipment used in the procedures.
3.Ability to circulate, assist, monitor, and document hemodynamics and electrocardiograms during the procedure.
4.Knowledge of and ability to prepare intra-aortic balloon catheters.
Assignment. Technicians at this level aid the staff with procedures which are routine and standardized in nature.
They explain the procedure to the patient to secure the patient's confidence and cooperation; position patients for procedures; and restock linen and consumable supplies. They set-up, clean, and ensure proper performance of the endoscopic equipment; and prepare the procedure rooms for turn-over, adhering to universal precautions and SOPs. The technician logs procedure and reprocessing data, adhering to patient privacy and information security policy. Assignments at this level require knowledge of basic medical terminology as well as common gastrointestinal anatomy.
Experience: Must meet basic requirements and possess at least one year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, abilities, and other characteristics related to the duties of the positions to be filled. In addition, the candidate must demonstrate the following KSAs:
1.Ability to assist with insertion of temporary transvenous pacemaker catheters.
2.Ability to complete individual cases by pulling sheaths and holding pressure to obtain hemostasis.
3.Ability to assist with closure devices.
4.Ability to appropriately educate patients and patient families regarding procedures and post?-catheterization care.
Assignment. This represents the full performance level.
Technicians at this level function with minimal instruction, assisting the staff with complex specialized procedures such as ERCP and endoscopic ultrasound (EUS). They are proficient with GI endoscopic equipment and ensure proper performance of endoscopic equipment; select alternate equipment or adjust equipment settings to accommodate patient needs and produce high quality technique. They collect, process, and label all specimens; train other technicians in basic endoscopic procedures; work in and outside the GI unit offering technician support during emergent and on-call endoscopic cases; and assist the nurse in patient care duties such as obtaining vital signs, dressing and documenting all pertinent information in the patient's records. They inventory and maintain par levels of specialty equipment for the endoscopy unit.
Experience. At least 1 year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, abilities, and other characteristics of the positions to be filled. In addition, the candidate must demonstrate the following KSAs:
1.Ability to scrub, circulate, and monitor advanced procedures (including electrophysiological) to include rotoblation, intravascular ultrasound, RADI or wave-wire, angiojet, filter wires, myocardial biopsies EPS, permanent pacemaker insertions, AICD insertions, and biventricular pacemaker insertion.
2.Advanced knowledge of equipment, medications, and supplies used in the Cardiac Catheterization Laboratory and the ability to set-up, calibrate, operate, and troubleshoot this equipment.
3.Knowledge and understanding of wave forms from the great vessels and all chambers of the heart including the ability to recognize and distinguish normal from abnormal, artifact from abnormality, and forewarn the physician of an impending life-threatening situation.
4.Knowledge of quality control and performance improvement indicators for the Cardiac Catheterization Laboratory and ability to prepare and analyze quality control of test results.
Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and range of variety, and be performed by the incumbent at least 25% of the time.
