The Executive Medical Director is responsible for oversight of care management, population health, quality, patient safety, risk management, and utilization initiatives implemented within the regional Joint Venture (JV). Atlantic Coast Health Network is a Joint Venture between Holy Cross Hospital, subsidiary of Trinity Health and Memorial Healthcare System. A Regional Clinically Integrated Network currently managing 160,000 patient and with the participation of approximate 1680 providers. The Executive Medical Director reports directly to the ACHN JV board of directors and will collaborate closely with the executive director of the organization. Maintains close working relationships with physician leaders at partner organizations to promote clinical quality initiatives. Identifies and promotes best practices to improve quality and outcomes and control costs across the network. Serves as clinical leader in support of network development, clinical integration, and population health management. Coordinates with Information Technology in the establishment of IT solutions to support population health and care management functions and assess these functions against clinical quality, outcomes, and total cost of care. Interacts and develops relationships with payer partners and local employers in relationship with ACHN value proposition.

ESSENTIAL FUNCTIONS

  • Knows, understands, incorporates and demonstrates the Trinity Health Mission, Vision and Values in behaviors, practices and decisions.

  • Provides the direct clinical direction for ensuring the high quality and efficient care of the regional network. This includes development of care plans, working with other physician members of the care team to ensure the desired outcome.

  • Serves as the primary physician in the performance of and leadership of utilization management functions.

  • Provides leadership for and oversight of care management, population health, quality, patient safety, risk management, and utilization initiatives implemented within the Joint Venture.

  • Sets direction and goals for the performance assessment metrics related to the care model, population health, and total cost of care for the Joint Venture.

  • Builds and maintains close working relationships with physician leaders and other key contacts at partner organizations to promote clinical quality initiatives.

  • Provides leadership over and oversees key operational aspects of quality, care management, care coordination, and process improvement initiatives.

  • Identifies and works to spread best practices and standardized care to improve quality and outcomes and control costs across the continuum.

  • Oversees development of educational content for physicians and physician organizations around care management and population health management.

  • Defines and implements clear processes to evaluate and address care management gaps in the network, and develops programs to address these gaps as necessary.

  • Works collaboratively with Executive Director and CIN/ACO leadership to minimize unnecessary duplication of resources.

  • Works on behalf of ACHN Population Health to meaningfully integrate population health strategies at within the participating networks

  • Works closely with the Executive Director and the regional leads for network development to build and maintain relationships with the physician organizations that do or may partner with the Joint Venture.

  • Serves as clinical leader in support of network development, clinical integration, and population health management.

  • Coordinates with IT to provide oversight of the development, implementation and monitoring of the IT solutions necessary to support population health and care management functions, as well as the assessment of these functions against clinical quality, outcomes, and total cost of care.

  • Ensures HIPAA compliance.

  • Establishes and manages patient complaint evaluation and follow-up processes as they relate to performance against Network standards.

  • Reviews customer satisfaction surveys and develops action plans as appropriate.

  • Tracks industry trends and standards for group practices and for clinically integrated networks (CINs).

  • Oversees and manages any staff deemed appropriate by the Board

  • Works with the ACHN Core executive leadership to establish and manage against multi-year strategic plan related to clinical quality, outcomes, and total cost of care as approved by the Board.

  • Participates in regional CIN/ACO decision-making and work groups impacting Population Health, Physician Networks, Quality, etc. Assumes leadership in implementing initiatives within the JV.

  • Knows, understands, incorporates and demonstrates the Joint Venture mission and vision in leadership behaviors, practices and decisions.

  • Collaborates with and provides support to other employees of the Joint Venture as needed.

  • Conducts special projects as requested by core JV executive leadership.

  • Performs all other duties as assigned.

MINIMUM QUALIFICATIONS

  • MD or DO degree from an accredited medical school.

  • Board certified physician with medical license in good standing.

  • Ten (10) or more years of clinical practice experience in a hospital or health care system including experience with implementation of clinical transformation, care management, and population health management initiatives. Five (5) or more years of clinical leadership and medical management experience preferred.

  • Knowledge and understanding of the healthcare environment and full continuum of healthcare options, programs and services.

  • Knowledge of related governmental and accreditation agency requirements.

  • Working knowledge of physician and/or provider based contracting and care management models.

  • Strong understanding and proficiency in the use of metrics and measures in managing programs and services.

  • Demonstrated leadership and management skills.

  • Excellent interpersonal and communication skills.

  • Excellent oral and written communication and presentation skills.

  • Strong interpersonal skills with proven ability to initiate and cultivate productive collaborative relationships with physician leaders at partner organizations. The incumbent is also required to communicate effectively with physician providers to positively affect behavior and function as the primary physician liaison on care management related matters.

  • Experience as an active listener and a change agent, with demonstrated ability to build consensus.

  • Experience working with clinical information systems supporting population health management preferred.

  • Proven success in working in a large complex organization with matrix reporting relationships.

  • Ability to maintain a high level of satisfaction with referring physicians/institutions and members.

  • Must be comfortable operating in a collaborative, shared leadership environment.

  • Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and to motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.

  • Must be able to travel to the various JV owner /partner organization sites, both local and distant, as needed.

The GWHP Regional Medical Director is accountable for the clinical and pharmacy program strategy for GWHP and will work closely with the GuideWell enterprise medical operations team to execute the strategy. This role serves as the primary point of contact and is accountable for ensuring the anchor provider partners are able to deliver on the Plan's accreditation, utilization and quality targets. The Regional Medical Director will serve as the physician champion for our anchor provider partners and ensure they are fully leveraging the suite of population health analytics to manage their patient populations. Because of the unique structure our GWHP's operating model, this role requires a high degree of coordination with external and internal business partners, including, but not limited to the providers, health care systems, Prime, Wellness, appeals and grievance, Quality, Case and Disease management, and other vendors who provide care management services to the enterprise. The GWHP Regional Medical Director must possess a solid knowledge of how care management strategy impacts network, customer experience, provide experience, medical costs, provider risk arrangements and the broader value proposition of the GWHP solutions in the market – with an emphasis on affordability. Job duties include but are not limited to: * Responsible for developing the strategic plan and direction for GWHP's care and pharmacy operating model. This includes managing the execution of the operating model with leaders within the medical operations area * Develops innovative approaches to meet organizational goals and objectives while effectively managing change and administrative costs * Strategize with the GWHP leadership team and other GW leaders regarding program expansion, business development, new care/pharmacy models and establishing appropriate clinical and business targets * Monitor marketplace trends and gather competitive intelligence * Serves as the primary point of contact and is accountable for all aspects of health plan clinical/pharmacy and utilization management performance * Work collaboratively with the provider partners and key health care systems to support achievement of quality initiatives; develop, implement/execute care programs, monitor outcomes of the planned initiatives and adjust tactics as needed to meet targets * Provides leadership, coordination, and direction to achieve effective clinical/pharmacy programs and operational excellence, while focusing on member and provider outcomes * Work closely with the GWHP team to deploy new collaborative provider relationships that recognize and reward improved quality and cost efficiency.
