Key Responsibilities:

  • Conduct ongoing assessments of clients' personal strengths, progress, challenges, and needs

  • Provide individualized assistance to clients in accessing financial, medical, and housing resources

  • Develop and implement critical intervention plans to address unique challenges

  • Maintain regular contact with hospitalized clients and coordinate their care with healthcare providers

  • Complete required charting and documentation, ensuring compliance with operational guidelines

  • Meet service and documentation requirements for programs such as Ryan White and Greene County Health Department

  • Assist clients in adhering to prescribed medication regimens

  • Facilitate access to mental health care for clients as needed

Education and/or Experience Qualifications:

  • Bachelor's Degree in Social Work, Nursing, or a related field (Master's degree preferred)

Additional Qualifications:

  • Demonstrated experience working with adults with HIV/AIDS

  • Valid driver's license, clean driving record, and reliable transportation

  • Ability to maintain accurate and up-to-date records subject to MDHSS review and inspection

  • Passion for making a real difference in the lives of individuals living with HIV/AIDS
