About the Childrens Healthcare Center

Our Childrens Healthcare Center provides care for medically fragile and/or technology-dependent children, ages newborn through 21 years. Most children are admitted from a hospital NICU or PICU. This program focuses on patients who no longer require acute care, but who need significant medical attention related to wound therapy, PICC antibiotics, post-surgical care, NG, GJ tube feedings, and ventilator care. The Center is a home away from home for medically fragile children and their families.

About the Medical Case Manager Position

The Medical Case Manager reports to the Director of Nursing at the Childrens Healthcare Center (CHC), and is responsible for the collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for the clients in our care. She/he provides services to meet the clients and his/her family's comprehensive health needs through communication and resource connection to promote quality cost-effective outcomes. The Medical Case Manager is responsible for collaborating with referral sources and insurance companies to reach the maximum census at the CHC.

Essential Functions

  • Use a patient-centered, collaborative partnership approach as provided in the Individual Care Plan;

  • Facilitate self-determination and self-care through advocacy, shared decision making, and education;

  • Practice cultural competence with awareness and respect for cultural diversity;

  • Promote use of evidence-based care as available;

  • Promote optimal client and family safety;

  • Successfully link family and client with community resources;

  • Assist with navigating the healthcare system to achieve successful care (in accordance with the care plan) with an emphasis on transitioning the client to his/her permanent home;

  • Provide initial assessment and Individual Care Plan to appropriate third- parties (DSCC, DCFS or insurance companies);

  • Promote quality outcomes and assist in the measurement of those outcomes;

  • Support and maintain compliance with federal, state, local, the organizations, and program certification rules and regulations.

  • Conducts a comprehensive assessment of the client's health and psychosocial needs on intake, including health literacy status and deficits, and collaboratively develops the Individual Care Plan with the interdisciplinary team (lDT);

  • Coordinates care with the client, family, caregiver, primary care physician/provider, other healthcare providers, the payer and community to achieve efficient and effective outcomes.

  • Maintains important relationships with the discharge planners and referring personnel at the childrens hospitals and units in the Chicagoland area;

  • Facilitates communication and coordination among the IDT in order to minimize fragmentation of services;

  • Educates the client, family and/or caregivers, and members of the lDT about treatment options, community resources, insurance benefits, psychosocial concerns, etc. so that timely and informed decisions can be made;

  • Assists in the marketing of the agency and CHC to referral sources, third party payers, and outside agencies;

  • Empowers the client/family to explore options of care, when available, and alternative plans, when necessary, to achieve desired outcomes;

  • Assists the client and family in safe transitioning of care to the next most appropriate level;

  • Strives to promote client self-advocacy and self-determination if possible;

  • Prepares the client and the family for discharge, coordinating necessary resources;

  • Performs public relations and marketing assignments tasks as assigned;

  • Plans and carries out family support meetings;

  • Performs other related duties as assigned.

Minimum Requirements

Education/Experience:

  • Bachelors Degree in nursing or social work required, Masters preferred;

  • Experience with case management preferred;

  • Networking abilities as demonstrated by work experience with Chicagoland childrens hospital units;

  • Bilingual (Spanish speaking preferred).

Skills/Knowledge:

  • Understanding of pediatric disease and illness, psychosocial interventions, and child welfare mechanisms;

  • Excellent verbal, written, and interpersonal communication skills;

  • Understanding of Maryville CHC Program and mission;

  • Proficiency with MS Windows and Office;

  • Experience with an electronic healthcare record system preferred.

Physical Demands:

  • Required to frequently sit, stand, bend, stoop and walk for extended periods;

  • Required to lift and move objects weighing in excess of 40 pounds;

  • Must have visual, hearing, and learning capabilities sufficient to perform the essential functions as defined in the job description.

Environment/Scheduling:

  • Work is performed in designated program and clinical settings;

  • Duties may involve contact with clients and family members who may be threatening, aggressive and/or under the influence of drugs or alcohol;

  • Must be flexible in scheduling, which includes weekend availability for crisis situations, and must be available to work additional hours when necessary;
