Job Summary

Grant Funded Position

Under limited supervision of the Manager, Medical Case Management, this job is responsible for the provision and evaluation of services delivered by assigned Medical Case Managers for delivery of competent, quality patient care and support services for assigned areas. All team members are expected to be knowledgeable and compliant with Prisma Health's values of compassion, dignity, excellence, integrity, and teamwork.

Accountabilities

• Manages a defined case load while performing the duties of Team Leader.

• 50%

• Supports the Department Manager by providing leadership and mentoring guidance to assigned staff.

• 10%

• Serves as Team Leader, coordinating the work of a group of assigned staff in the day-to-day operations of the team.

• 15%

• Assist with hiring and training of new medical case managers.

• 10%

• Assist with Social Work Intern training and supervision.

• 10%

• Oversee operations of MCMs in the absence of the director per request of director.

• 5%

Minimum Requirements

• Master's Degree

• Social Work, Counseling or health/human services related

• 3 years

• Ambulatory or Acute care setting

Knowledge, Skills or Abilities

• Must possess knowledge and skills to provide clinical and administrative leadership to assigned team. Knowledge of Healthcare Reform, government and third party reimbursement systems and contracts. Strong organizational, time management, problem solving, and critical thinking skills. Firm understanding of pathways, procedures, and governmental regulations. Competent knowledge base representative of the skills and training of a MSW Case Manager.
