Position Title: Medical Care Coordinator
Department: Nursing
Reports To: Director of Nursing
Entry Level Qualifications:
High School Diploma or GED required. College degree preferred.
Minimum of 1-year healthcare experience. Prefer 2 years of healthcare experience

8.Must be able to adhere to the practice of confidentiality with regards to patient information and confidential corporate information including but not limited to, confidential information involving clinical research.
9.Must be able to operate basic office equipment necessary to perform job duties including Microsoft Word, Excel, and Microsoft Office.
Maintains professional integrity throughout the performance of all job duties.
Must be able to type at least 50 wpm.
If diagnosed with an addictive disorder, must have a minimum of two years of sustained remission.
Responsibilities:
The medical care coordinator will assist the MAT Nurse in the coordination of patient care and the management of the medical provider team.
The medical care coordinator will assist the MAT Nurse in the administration and supervision of addiction medicine fellows, medical residents and medical students.
The medical care coordinator will perform prior authorizations with third party payers for medication assisted therapies used in the treatment of substance use disorders.
The medical care coordinator will know and abide by the general provisions of CFR 42 Part 2 "Confidentiality of Drug and Alcohol Abuse Patients Records" and "Health Insurance Portability and Accountability Act". This position has unrestricted access to patient identifying and patient health information.
Key Functions:
1.Participates in program planning for the continuum of care including discharge planning for MAT services.
2.Facilitates follow up on the status of all commercial insurance for MAT.
3.Coordinates injectable MAT preparations with the medical team.
4.Responsible for shipments and storage of injectable forms of MAT and the associated management of co-pays and deductible payments.
5.Working with the medical director and the discharge planning team to arrange the seamless transition of MAT care from the inpatient setting, into the outpatient setting.
6.Work with specialty pharmacies in the ordering of the injectable forms of MAT.
7.Process all patients on the daily list. This will include submitting MAT treatment plans to insurance companies for approval.
8.Follow up daily on the status of all commercial insurance that is in the process of approval.
9.Other duties as assigned.
