As a member of an integrated care team, works directly under the supervision of a registered nurse or physician, providing services to patient in homes, facilities and via telephone. Duties include hands on care, clerical support, and telephonic interventions.

The pace is fast and you'll need to be comfortable managing multiple priorities. You must also be able to respond calmly and effectively.

Primary Responsibilities:

  • Provide support to Physicians, Nurse Practitioners and Registered Nurses to help facilitate medical activities for multiple complex health patients

  • Manage provider orders, patient medical records, medication refills, scheduling appointments, and coordination of care

  • Participate in patient - centered field visits to patient homes to assist providers for clinical tasks including, but not limited to: venipuncture, lab collection, pulmonary function tests, and vital signs

  • Collaborate with both internal TeamMD providers and external provider organizations to ensure patient centered care plans are agreed upon and respected

  • Must possess the ability to think outside the box when addressing patient - centered solutions to non - standard request and problems

  • Work in a self - directed manner, while demonstrating an understanding of when and how to consult with other team members to address complex needs

  • Communicate effectively with medical, professional, administrative and other personnel to effectively present information orally and in writing

  • Demonstrates accurate and thorough documentation skills and compliance with documentation guidelines

  • Provide clerical and telephonic support including but not limited to confirming providers schedules, must demonstrate the ability to triage emergent patient and pharmacy phone calls

  • Ability to possess open communication and to help RN coordinate with OT, PT, ST, Case managers and other providers involved in the patient's plan of care

  • Actively participates in weekly care - coordination and monthly IDT meetings

Required Qualifications:

  • High School Diploma / GED (or higher)

  • Certified Medical Assistant OR 2 years of working experience as a Medical Assistant

  • 2 years' clinical experience

  • 1 year of working in medical practice

  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area

  • Maintains strict confidentiality regarding knowledge of all patient information (HIPAA), staff and confidential business

Preferred Qualification:

  • Experience working with people with complex health care and social needs

  • Previous Experience with Phlebotomy

  • Current CPR certification

  • Strong problem - solving skills with the ability to work collaboratively within a team to find patient - centered solutions for complex challenges and barriers

  • Demonstrates the ability to observe, record and report patient's needs, concerns and coordinates the needs of patients to maintain constructive, healthy relationships with them and all care providers

  • Ability to objectively and compassionately address patient needs in a responsive manner.

  • Ability to work with diverse care teams

  • Ability to think creatively and comfort being hands - on in a fast - paced, entrepreneurial environment

VIEW JOBS 4/18/2019 12:00:00 AM 2019-07-17T00:00 Position Type: PRN Scheduled Hours per 2 week Pay Period: 0 Primary Location: IA > WEST DES MOINES > MERCY WEST LAKES GENERAL SUMMARY: Provides technical support in the delivery of pharmaceutical care to clients of Mercy Medical Center's Outpatient Pharmacies. Technical support includes the preparation and delivery of medications and stocking and ordering of drugs and other products. Technical work is performed under the supervision of a licensed pharmacist. ESSENTIAL FUNCTIONS: * Prepares patient drug orders as follows (with proper checks performed by a pharmacist as required by law): enters patient prescription orders into the computer; selects correct medication, strength and dosage form; prepares labels; selects and affixes auxiliary labels to medications as appropriate and legally required; counts, measures, weighs & pours medications; documents doses dispensed; files medication orders. * Cleans counters, medication drawers, carts, shelves, and equipment as needed. * Assists the pharmacists with audits (e.g. allergy, weight, etc.). * Enters and fills stock bags and fill stock requisitions for other departments. * Accurately processes and restocks medications for departments utilizing automated dispensing systems, i.e. Pyxis/Acudose. * Performs a variety of medication inventory-related tasks including: communication of stock utilization and ordering needs; assisting with reordering; checking of expiration dating with alerts to pharmacists when needed; and rotating and restocking. * Compounds and labels extemporaneous liquid and solid products for patients according to procedure. * Answers telephone and process requests according to procedures. * Participates in the training of new technicians. * Charges and credits individual patient accounts. * Accurately and efficiently waits on patients at the POS register and handles sale transactions. MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: * High school diploma or GED * Registered or eligible to register as pharmacy technician with the State Board of Pharmacy Examiners within 30 days of hire. * Pharmacy Technician Certification Program required.
