Responsibilities:

  • Collaborate with care team members to enhance patient access, reduce emergency visits, and lower readmissions.

  • Conduct telephonic outreach to discharged patients for follow-up appointments and medication reviews.

  • Make referrals to the extended care team and notify providers of post-discharge needs or discrepancies.

  • Participate in population health outreach and assist in closing care gaps for patients.

  • Provide health education to promote healthy lifestyles and enhance patient engagement through self-management support.

  • Utilize motivational interviewing techniques to improve patient health literacy and engagement.

  • Guide workflow and assign tasks to clinical staff, assisting in supervisory activities and training for new employees.

  • Serve as the Epic Care super user and collaborate with management to identify improvement opportunities in clinical functions and infection control.

  • Organize routine staff meetings to communicate procedural changes for enhanced service quality and patient safety.

  • Ensure compliance with HIPAA regulations when handling patient records and documents.

  • Manage patient charts, schedule appointments, and verify insurance coverage and demographics.

  • Prepare patient rooms, set up equipment, and maintain infection control protocols.

  • Assist patients with mobility and collect vital information, while relaying instructions from healthcare providers.

  • Perform lab tests, including phlebotomy and EKGs, and document patient information accurately.

  • Comply with regulatory agency requirements and maintain confidentiality in handling medical records.

  • Administer medications and vaccines under physician supervision, while adhering to UPMC Health System policies.

Qualifications:

  • Successfully complete the UPMC preceptor training and serve as a preceptor and mentor to newly hired team members

  • Must be responsible and demonstrate proficiency in all practice category specific tasks

Required:

  • High school diploma or equivalent

  • Successful completion of Medical Assistant program, or equivalent hospital program, OR

  • Successful completion and maintenance of certification examination and 1 year of patient care experience, OR

  • Two years of direct patient care experience

Licensure, Certifications, and Clearances:

In lieu of the MAS, relevant bachelor's degree accepted.

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

  • Act 31 Child Abuse Reporting with renewal

  • Act 33 with renewal

  • Act 34 with renewal

  • Act 73 FBI Clearance with renewal
