How will a candidate be successful in this role?

Under direct supervision of the staff RN, provides professional patient care by assessment and evaluation of patients in a timely/efficient manner, while communicating information to health care providers, both written and verbal in an accurate organized fashion.

Job Duties and Responsibilities:

  • Completes routine provider appointment check-ins and rooming of patients, to include vital signs and medication reconciliation.

  • Coordinates care between other providers, including obtaining ROIs and requesting records.

  • Completes Urine Drug Screens, breathalyzers, and pregnancy tests as needed.

  • Other duties as requested by assigned provider or RN, within scope of practice

  • Transports patients to outside appointments and accompanies patient into the appointment as needed to assist with coordination of care.

  • Acts as liaison between outside providers and CRC treatment team.

Other Functions:

Performs other such duties as assigned within the scope of the position and program description, as well as those reflective of their experience, education and ability.


  • High School Diploma or GED.

  • Medical Assistant Certificate

  • One year of directly related experience.

  • Knowledge of the Medical Field.

  • Medical Terminology.

  • Good computer and typing skills

  • Communication, organization, time management, and clinical skills.

  • Must have valid driver's license and maintain a good driving record while employed with Community Reach Center's Programs.
