The Meadows Skilled Nursing Facility is seeking a
Full Time MDS/MMQ Nurse to join our team!
The MDS/MMQ Nurse is responsible for the development and completion of the resident assessment and care planning in accordance with state and federal requirements, and policies and procedures of the facility.

Current, valid, and active license to practice as a Registered Nurse in the state of employment required.
Three (3) years of experience in a healthcare setting required.
Current BLS certification from a Vibra-approved vendor required.
One (1) year of skilled nursing and/or rehabilitation experience preferred.
One (1) year of experience in MDS/MMQ Software preferred.
Ability to project a professional image.
Knowledge of regulatory standards and compliance requirements.
Strong organizational, prioritizing and analytical skills.
Ability to make independent decisions when circumstances warrant.
Working knowledge of computer and software applications used in job functions.

Per Federal Health Care Worker Vaccine Requirements, all employees must be fully vaccinated against COVID-19 or be approved for a valid religious or medical exemption.
