You love what you do, now love where you do it. Responsibilities Provides required Minimum Data Set (MDS) assessment of all residents in facility. Participates as an active member of the ID Team.

Works with the DON to provide accurate information for quality care of the resident including audits and care conferences Essential Accountabilities:• Assists with completion of MDS assessments and all required documentation; admission, quarterly, annual and changes of condition.• Participates in follow-up audits to assure corrective action to identified problems.• Assist nursing department in developing appropriate corrective action to identified problems.• Participate as a member of the ID Team.• Support the DON, DSD and Nursing supervisors through communication and utilization of necessary tools to achieve accurate resident assessment resulting in quality care.• Attend meetings as requested.• Insure all areas on MDS are completed prior to transmission to the State.• Assists in entering physician orders into the computerized program.• Assists in completion of care plans as directed by nursing staff and MDS Coordinator.• Performs other duties as assigned. Knowledge, Skills and Abilities:• Long term care regulations• Standard administrative and secretarial practices• Proper formats for a variety of correspondence, reports and other documents• Assigned department, division program, practices and terminology and role in organization• Standards and protocols for record and file organization and maintenance• Intermediate to advanced skills in word processing, presentation and spreadsheet tools• Intermediate to advanced level operation of graphics and presentation software• Demonstrated superior level of customer service• Correct English usage in spelling, grammar, punctuation and vocabulary• Read and comprehend office or program manuals and reports• Compose correspondence, instructions and reports using appropriate formats and styles Qualifications Education• High School Diploma, General Education Development (GED) or Completion of a CMC Approved Individualized Education Plan (IEP) Certificate required• Completion of an accredited Vocational Nursing School program OR become licensed by equivalency required Experience• 2 years of Long Term Care experience required• Experience in completion of MDS assessments preferred Licenses and Certifications• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required• LVN - Current State of California Licensed Vocational Nurse License required Disclaimers Pay ranges listed are an estimate and subject to change.• If any bonuses are noted, they are only applicable to external hires meeting criteria.
