SUMMARY OF MDS EXCHANGE COORDINATOR POSITION
Frontline Management is seeking an MDS Exchange Coordinator to join our team in Lafayette, CO. The ideal candidate for this role will be a driven self-starter who has a genuine interest in working in a collaborative team environment. We strive to provide care to our residents that exceeds industry standards and we are looking for an MDS Exchange Coordinator that wants to join that mission!
The MDS Exchange Coordinator coordinates the MDS Process (MDS, CAAs and Care Plan Recommendations) remotely for all residents in assigned/directed communities by performing all Medicare and Non-Medicare Minimum Data Sets.
Oversee and coordinate the development and completion of the resident assessment (MDS) remotely in accordance with current Federal and State rules, regulations, and guidelines that govern the resident assessment, including the completion of assigned CAAs.
Assemble information from the Initial Nursing Assessment, Baseline Care Plan, and clinical record review to complete the nursing portion of the Minimum Data Set.
Monitor and follow-up with facility team members as needed to verify that all assigned sections of the MDS are completed, dated, and signed within designated time frames.
Review each MDS for consistency, completeness, and signatures prior to submitting
Valid and active state-issued Registered Nurse (RN) license.
Two years of experience in long-term care preferred.
Must possess sound knowledge of nursing principles, procedures and standards of practice as determined by state and federal regulations.
Ability to read, analyze, interpret, effectively present information.
