Job Summary:

Responsible for the planning, organizing, evaluating, directing and supervising the overall operation of the MDS system documentation in accordance with current applicable federal, state and local standards, guidelines and regulations that govern the facility.

Essential Duties and Responsibilities:

  • Respond to all staff, family members and residents in a courteous, patient and caring manner

  • Respect the dignity of each person

  • Communicate in a positive and helpful manner

  • Follow established performance standards and perform duties according to policies and procedures

  • Coordinate all MDS assessments and timeliness of documentation.

  • Delegate responsibility for completing assessments to MDS Coordinator's.

  • Perform MDS Coordinator functions as needed and specific to caseload.

  • Educate staff involved in MDS documentation.

  • Attend and participate in workshops and seminars to keep informed of current changes in the health care field especially specific to PPS, RAI/MDS.

  • Conduct audits on MDS documentation and forward report to Director of Q.A.

  • Review monthly reports from State.

  • Electronically submit MDS information to State; maintain permanent file on all electronic submissions.

  • Review and evaluate work performance of MDS Coordinators issue, discipline any action of necessary and counseling. Develop PIPs as needed.

  • Interview and train new MDS Coordinators.

  • Attend meetings as requested such as Daily PPS, Weekly UR, Monthly Triple Check.

  • Approve time off and vacation schedules.

  • Update MDS policy and procedures as needed.

General Requirements:

Effective management of time and schedule constraints is necessary. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Education/Experience:

  • Graduate of an accredited school of nursing and currently licensed RN in the State of Massachusetts.

  • Minimum three years' experience in long term care facility.

  • Work experience in MDS documentation.

  • Computer experience essential.

  • Communication Skills:

  • Effective written and oral communication skills and organizational ability.

Job Summary: The MDS/PPS Coordinator is responsible for the effective implementation of the overall PPS process, including timely and accurate completion and transmission of MDS's. Manages the Medicare residents from pre-admission to discharge. Insures appropriate documentation to support skilled coverage. Full Time 5 Days Per Week Essential Duties and Responsibilities: * Schedules MDS assessments per guidelines, as assigned and maintains a monthly calendar. * Informs members of the Interdisciplinary Team with the ARD and MDS's due. * Schedules Care plan meetings as needed and notifies Interdisciplinary Team Members of schedule. * Notify members of the IDT of changes or additions to monthly Care Plan Meeting schedule, for new admission, change of condition or interim meetings. * Contribute to the Interdisciplinary care plan by communicating any resident issues that are pertinent to the plan of care. * Review charts and complete all MDS for assigned residents. Conduct interviews as assigned, with residents and staff (according to MDS guidelines) to ensure that the information captured is accurate and timely. * Identify and communicate need for Significant Change in status assessments. * Complete MDS/CAAs timely per schedule. * Electronically submit MDS information. Review, revise and correct, and resubmit as appropriate. * Complete Discharge Assessment and entry/re-entry tracking forms per Federal regulations. * Audit MDS assessments and CAAs from all disciplines and notify discipline of any discrepancies. * Assist with necessary completion of the nursing portion of the PPS MDS/CAAs in the absence of the Medicare MDS Coordinator as needed. * Review the ADL flow sheets and communicate with nursing and C.N.A.'s when discrepancies are found and ensure that corrections are in place. * In collaboration with the Medicare MDS Coordinator and Staff Development, orient and train staff to the RAI and care planning process to assure compliance with State and Federal Regulations. * Maintains a viable working relationship with all disciplines involved in the planning of resident care. * Participates in facility QA initiatives. Education/Experience: * Graduate of an accredited school of Nursing and current MA License required. BSN preferred. Five years' experience in geriatric nursing required. Two years' experience as an MDS Nurse and AANAC certification preferred. Must be well-informed about long term care reimbursement and have knowledge and training in all aspects of the scheduling and completion of the MDS and CAA processes. Pertinent job-related computer skills required. SNF Medicare experience preferred. Must possess the ability to deal tactfully with staff members, residents, family members, visitors, government agencies and the general public.
