Sierra Health Care Center is seeking a qualified and experienced MDS Coordinator. Sierra, located in Truth or Consequences, is a leader of the next generation of skilled nursing and long term care services. With beautiful Elephant Butte Lake nearby, this established senior campus, has been providing exceptional assisted living, skilled rehabilitation, long term care, home health, and hospice services to the heart of southern New Mexico for over 30 years.

Duties & Responsibilities:

Schedule residents for assessment per federal and state requirements
Develop, complete and transmit an accurate MDS for residents within specified schedules and federal and state requirements
Participate in Interdisciplinary Care Plan Meetings to coordinate care needs identified in the MDS
Assist the Director of Nursing in Planning, developing, organizing, implementing, evaluating, and directing the nursing service department, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern nursing care facilities
Monitoring of resident care and services to achieve quality outcomes and high customer satisfaction

Requirements

Qualifications & Requirements:

Current Nursing licensure as an RN or LPN
Proven MDS experience, preferably one year minimum
Detail-oriented
Strong Organizational and Assessment Skills
Experience in skilled nursing/or sub-acute care setting
Experience with MDS development, completion and timely transmission or similar process in another health care setting
Critical thinking and problem solving abilities
Strong computer skills
Ability to meet regulatory deadlines
Enjoys a brisk and energetic work pace

This individual will work closely with our corporate Director of Nursing Education to provide continuing education for our existing nursing staff and assist with training new staff members to provide excellent skilled nursing care for our patients and residents. Selective Placement Factors Successfully completed the Primary NWCG Core Requirements of Division Group Supervisor (DIVS) and Secondary NWCG Core Requirement of Incident Commander Type 3 (ICT3) or Prescribed Fire Burn Boss Type 2 (RXB2) OR Primary NWCG Core Requirements of Air Support Group Supervisor (ASGS) and Secondary NWCG Core Requirement of Incident Commander Type 3 (ICT3) -AND- Successfully completed the NWCG Course # M-581 Fire Program Management A minimum 90 days experience performing on-the-line (Primary/Rigorous) wildland fire suppression duties as a member of an organized fire suppression crew or comparable unit that utilized knowledge of wildland fire suppression, containment or control techniques and practices under various conditions. This experience must be documented with specific dates in the online application or resume. In order to be considered for this position, copies of your Incident Qualification and Certification System (IQCS) Master Record (or equivalent training documents) which contain documented proof of the certification or attainment of the IFPM Selective Placement Factor for this position MUST be attached to your application.
