Coordinate the RAI Process. Work in Collaboration with the interdisciplinary team to assess the needs of the patient/resident Significantly involved in Care Coordination meetings Ensures accurate and timely MDS assessments according to state and federal regulations. Ensures and completes accurate coding of the MDS assessment with information obtained via medical record review, observation and interview with center staff, patients, residents and family members. Maintains the tracking system of MDS assessment schedules (time frames and due dates). Coordinates Care Plan conferences with the interdisciplinary team, patient, residents and families. Obtain, review and maintain all State and Federal reports, making appropriate corrections timely. Monitors Quality Measures and ensures that MDSs are accurate to support and reflect the Quality Measures. Provides education related to the RAI Process Coordinates and completes electronic submission of required documentation to the State data base and other entities per company guidelines and State and Federal regulations. Ensures Medicare and Medicaid regulatory guidelines are completed accurately and timely (i.e.: certifications, denial letters, skilled documentation, coverage criteria, etc)


Two years of MDS experience preferred, but not required Must hold current registered nursing license in the state of employment. Experience in Long Term Care preferred.

Computer skills such as Data Entry and Word processing required. Working knowledge of the MDS 3.0 (current version of minimum data set). Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to skilled nursing facilities. Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care.

In coordination with the Vice President of Philanthropy Operations, this position will coordinate philanthropy programs and campaigns to include the cultivation, solicitation and stewardship of grateful patients, caregivers and other constituents. This position works under the supervision of the Vice President of Philanthropy Operations, Mosaic Life Care Foundation and employed by Mosaic Health Systems. Duties * Develops and coordinates all aspects of all giving programs (i.e. Grateful Patient Program, careGIVER Campaign, Giving Tuesday, Direct Mail and others) that help secure funding for Mosaic Life Care and needs identified by leadership. * Oversees donor recognition and stewardship efforts, including promotion and management of giving societies. * Provides support for the planned giving program, to include creation of the planned giving newsletter and other marketing materials. * Maintains a donor portfolio and supports the Constituent Relationship Management program at the discretion of the Vice President of Philanthropy Operations. * Coordinates internal communications (i.e. deliver philanthropy reports at staff huddles, communications meetings). * Assist with preparation of the budget to ensure necessary resources are available to carry out philanthropic plan. * Other duties as assigned Qualifications * All required education is a minimum requirement. Higher levels of education are acceptable. Bachelors in Business, communications, marketing, or other related field required. * 2 years relatable experience in a healthcare or non-profit setting required.
