Title: MDS Coordinator
FLSA Status: Full-Time, Exempt
Work Schedule: Monday - Friday; 8:30 - 5PM with weekend rotation
Rate of Pay: $83,200 - $116,168
GENERAL SUMMARY
The MDS Coordinator is an RN who is responsible for the overall supervision, coordination, maintenance and timely completion of the MDS, care plans and CAA's for residents. The MDS Coordinator acts as the Restorative Nurse and develops/maintains the restorative programming.

Essential Job Duties:
Coordinate and assist with completion and submission of interdisciplinary team, accurate, and timely MDS Assessments, CAAs, and Care Plans according to CMS RAI Manual Regulations
Report any changes in a patient's condition identified by the MDS Assessment to the DON
Provide education to direct care associates regarding updates or changes to the CMS RAI Manual or Skilled Nursing Facility Regulations that impact documentation
Assist with review of the Interdisciplinary Comprehensive Care Plan
Review Final Validation Reports and attest that all assessments have been completed and accepted into the CMSQIES system prior to billing and notify the Business Office when assessments are not ready to bill
Review CMS Reports to identify assessments completed or submitted late and develop systems and processes to prevent reoccurrence
Assures accuracy and completeness of entire medical record.
Updates care plans daily post event, new physician order, change of condition, etc.
Participates in family/resident care plan conferences.
Acts as the Restorative Nurse and develops, manages, and maintains the restorative programming.
Attend and participate in required meetings: Stand Up, Clinicals, PPS Meeting, Triple Check, QAPI, and other meetings as necessary
Assures effective communication with other shifts and departments as it relates to medical record documentation.
QUALIFICATIONS:
RN currently licensed by State of Illinois.
Current CPR certification. In- depth experience and knowledge working with MDS computer software.
Experience with the MDS transmittal process and understanding of regulations impacting the MDS. Supervisory experience preferred.
AANAC RAC-CT/A certification, or willingness to obtain within 3 months of hire
Restorative Nursing Certification, or willingness to obtain within 3 months of hire
