Who you are:
Current LVN Licensure by the California Board of Nursing. MDS certification preferred.
Degree from an accredited school of nursing, BSN, preferred
Minimum of 2 years geriatric nursing experience preferred
Minimum of 1 year of experience completing and submitting MDS
Current CPR/BLS certification
Job Summary:
Supported by the MDS Coordinator, assists with the completion of comprehensive assessments of residents upon admission, quarterly, annually, and for permanent change of condition. Supports the interdisciplinary team care plan conferences and monitors resident care plans for accuracy and timeliness of completion.
Essential Job Duties:
Assists the MDS Coordinator, and develops, and revises the MDS/care plan policy and procedure as needed.
Helps the MDS Coordinator complete MDS on each admission and keeps these current. Coordinates the automation and electronic transmission of MDS, following state/federal guidelines.
As assigned by the MDS Coordinator, contributes to initial care plans and monitors care plans on an ongoing basis for accuracy, completeness, and timeliness.
Assists the MDS Coordinator with monitoring the output of staff nurses responsible for completing assessments (admission, quarterly, change of condition, and annual).
Completes resident assessment as needed.
Supports follow-up of goals and plans developed at interdisciplinary care plan meetings.
Assists the facility in following state and federal regulations in regard to resident care and documentation.
Participates in monitoring the use of physical and chemical/psychotropic restraints closely, encouraging the lowest level of implementation that maintains resident safety.
Maintains open communication with all residents, responsible parties of residents, physicians, other staff members, and involved representatives of the community (friends of residents and staff of community agencies).
Attends in-service training classes and outside workshops and shares information with the Nursing Staff.
If assigned by the MDS Coordinator, attends Quality Assurance Performance Improvement meetings reports resident care issues and serves on QA subcommittees as assigned.
Attends rehab meetings, nutritional support meetings, and other meetings when assigned.
Any other relative, relevant duties as assigned by the MDS Coordinator or Director of Nursing.
