Job Description

Work Schedule: Weekend Schedule Friday-Sunday

Location: Presbyterian Hospital


M.D.-D.O. in a primary medical field appropriate to ABHPM standards. Five or more years experience. Prefer fellowship training in Hospice and Palliative Medicine. Board

certification in Hospice and Palliative Medicine expected within 2 years of employment.

Competencies and skills:

Assumes the attending role for hospice patients in the community and in Presbyterian hospice units, unless followed by their PCP.

As a consultant, works with patient and family to develop a recommended plan of care supports appropriate medical treatment in concordance with the patient's goals of care. Uses

clinical skills to relieve the suffering of patients and their families.

Provides support and oversight of the work of the hospice care team RN, APN or PA's direct patient care activities, as required by Medical Staff Bylaws.

Maintains coding and encounter data standards for completeness, accuracy, timeliness and reimbursement level including legible documentation. Provides full and appropriate

documentation of direct patient care services to support successful billing for all services.

HOME HEALTH RN The Registered Nurse provides skilled nursing care to patients in their home under the general guidance and direction of the Case Manager, according to the physician's orders and in accordance with Harmony Home Health policies and procedures, and following established standards of care. Essential Duties and Responsibilities: * Provide those services and/or treatments requiring substantial and specialized nursing skill. * Function as a team member demonstrating collaboration with, and responsibility to patient, physician, and interdisciplinary team. * Perform nursing care according to the needs of the patient and as indicated in the written plan of care. Assist in the assessment of needs, formulation and review of patient care plans.. * Have a good understand of Medicare guidelines.
