What we want
The GC-MOMS Program Specialist I, "Maternal-Child Health Navigator", under general supervision, performs entry-level specialist duties while providing comprehensive family and maternal-child health home visiting services to women and families participating in the Golden Crescent Management of Opioid Risk in Mothers (GC-MOMS) program.

Salary: Compensation will be commensurate to selected hire's experience.
Other Requirements or Factors: Requires reliable transportation (the program pays the federal/state rate for mileage). May require night and weekend work on occasion, including travel overnight for training and related activities.
Required Education and Experience:
Bachelor's degree in applicable field or equivalent combination of education and experience
No prior experience required
Required Knowledge, Skills, and Abilities:
Ability to multi-task and work cooperatively with others
Ability to communicate effectively, both in writing and verbally
Ability to convey health information clearly, actively listen, and collaborate with both community members and healthcare professionals
Strong interpersonal, organizational, and customer service skills
Maintains attention to detail and utilizes sound judgment
Ability to work a flexible schedule
Knowledge of word processing, spreadsheet applications, and MicroSoft Office programs
Must interact sensitively, effectively, and professionally with individuals from all backgrounds
Ability to travel throughout designated counties for program services and needs. (Victoria, Lavaca, DeWitt, Calhoun, Jackson County)
Preferred Qualifications:
Bachelor's degree in Public Health, Social Work, Human Services or related field
Bilingual in English/Spanish is highly preferred
Current CPR certification
Current Community Health Worker Training and Certification
Valid Driver's license or the ability to obtain one within 30 days of employment
Experience in maternal/child health, public/community health, mental health, home visits
Experience with Community Health Worker trainings and curriculum
Experience with outreach and public relations
Experience with additional Microsoft applications (i.e., PowerPoint, SharePoint, MS Lists, OneDrive, Visio, Planner, etc.)
Responsibilities:
Provides comprehensive home visiting services and health education to women and families participating in the Golden Crescent Management of Opioid Risk in Mothers (GC-MOMS) in accordance with Rural Communities Opioid Response Program
Health Resources & Services Administration (RCORP
HRSA). Maintains highest standards of home visiting in the context of healthcare and social services. Participates in Reflective Practice. Guides individuals to relevant healthcare and community resources, ensuring they access necessary services and support. Drives to homes of families participating in GC-MOMS to provide vital services. Carries caseload of up to 20 clients. Assists in planning, developing, implementing, coordinating, and monitoring GC-MOMS programs and services. Plans and develops specialized GC-MOMS program improvements such as conferences, seminars, lectures, meetings, and workshops to facilitate program goals. Assists in the development of policies and procedures. Follows organizational policies, procedures, and protocols and works to ensure all safety and regulatory guidelines are adhered to. Contributes to the mission, vision, and core values of the organization to promote a positive organizational culture. Follows attendance and punctuality rules. Demonstrates a professional attitude and employs professional ethics within working relationships.
