Responsible for the screening of potential new participants for PACE Lafayette. Responsible for the intake process of each Potential Enrollee. Responsible for completing and obtaining verification information for initial and yearly applications for the Medicaid Program.

Responsible for the entire enrollment process. Implements Marketing Strategies for PACE, including Health Fairs. Performs all intake screenings to initiate the application to PACE Lafayette.

Initiates the entire enrollment process into PACE Lafayette. Receives Intake telephone calls, explains the program, screens for eligibility criteria, completes Telephone Referral form. Schedules and coordinates intake activities.

Refers to other community resources as appropriate. Performs the intake visit on site and if necessary in the home. Ensures the potential enrollee has all necessary information concerning enrollment into PACE and prepares them for the assessment process, giving them all information and forms needed for the process.

Communicates with hospitals, nursing homes, physicians, and other community agencies for follow up in completion of the intake record. Provides follow up communication and correspondence to complete the enrollment process. Integral part of Interdisciplinary team with focus on giving information to staff, in order for team to make quality decisions for enrollment.

Completes enrollment activities and signatures for potential enrollees, interacting with Interdisciplinary team members and their care plans. Oversees and maintains the intake files and enters computer data. Complies with State Medicaid requirements for Initial and Annual Medicaid Applications within timeframe requested.

Responsible for initiating Medicaid applications and obtaining verification information for completion of Medicaid process.

Corresponds and communicates with DHH to complete necessary documentation to process applications. Communicates with Participants and Caregivers to complete the application process.

Builds and maintains effective collaborative relationships with area agencies and providers affiliated with the elderly and supplemental target markets. Functions as a Representative for PACE#Lafayette and Senior Services Division. Markets include, but are not limited to: physician#s offices, local and state government agencies, civic and business organizations, and health fairs.

Advanced ability in the use of computer software, correspondence skills, report writing and telecommunications in order to provide necessary marketing tools and provide information for Board Meetings and Senior Services Division. Description Minimum Required Preferred / Desired Experience # # 1 year experience working with the frail/elderly population. 2 years in a closely related health care field. # # Education # # Bachelor#s Degree in related field # # Training # # # # # Special Skills # # Must have computer skills. Microsoft work experience required.

Primary responsibilities include: * Perform pre-inspection checks for type of preventive maintenance required on vehicles * Keep accurate time and mileage logs * Tow trailers as needed * Drive and drop off vehicles and equipment to locations throughout the U-Haul Marketing Company area Requirements: * Valid driver's license and the ability to maintain a good driving record to operate commercial motor vehicles * Adhere to all local state and federal vehicular regulations while driving U-Haul drivers that operate service vehicles and/or rental trucks with a GVWR of 10,001 lbs. or more or a combination of vehicles (truck and trailer) with a GCWR of 10,001 lbs. or more are required to have a driver qualification file. A medical certificate from a federally registered medical examiner must be obtained to be a U-Haul CMV driver. Education/Training: High School Diploma or equivalent Work Environment: The work involves moderate risks or discomforts which require special safety precautions, e.g., working around moving parts or machines, fumes or irritant chemicals. May be required to use protective clothing, or gear such as masks, goggles, gloves or shields. Physical Demands: The work requires some physical exertion such as long periods of standing, walking, recurring bending, crouching, stooping, stretching, reaching or similar activities, and lifting a minimum of 50 lbs assisted or unassisted.
