Job Description:
Plans, organizes, directs, defines and coordinates clinical services necessary to meet the legal, organizational, medical staff and regulatory agencies guidelines.

Provide case management, education and discharge planning. Performs community and educational activities. Develops and provides a routine schedule insuring that all required components of the treatment schedule and patient care activities are provided at all times.

Provides clinical supervision to all members of the social service department of the inpatient behavioral health unit. Works with Program Leadership to establish and maintain standard treatment practice and coordinates the program scheduling with other department staff. Works with adolescent, adult and geriatric

Qualifications:

  • Experience required/preferred: (list required and preferred separately) 3 - 5 years of experience in an acute mental health hospital preferred

  • Licensure/Certification: (list required and preferred separately) Current Florida LCSW or LMHC License required.

  • Basic Life Support for Healthcare providers issued by the American Heart Association required.

  • Education required/preferred: (list required and preferred separately)Master's Degree in Social Work from an accredited school of social work required or Master's Degree as a Mental Health Counselor

  • Additional Experience: Able to conduct individual, group, family and marital therapy. Supervisory experience preferred

In addition to performing visits and completing coordination of client care, the Home Health Registered Nurse (RN) Case Manager is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role. Encompass Hospice RNs work with patients, their families, and other health care professionals during the final stages of patients' lives. Our nurses work in private homes, residential care facilities, nursing centers, and other hospice care environments. They may also supervise licensed vocational/practical nurses and nursing aides. Our nurses must be capable of compassionate communication with patients and families, have keen observation skills, high ethical standards, and knowledge of when to alert doctors and others about changes in patients' conditions.
