Responsibilities

Manages the Personalized Living department within a community. Ensures the efficient and effective operation ofthe Personalized Living department and interdisciplinary team process.

Provides direct management of all Personalized Living staff; to include coaching, counseling,disciplining and completing performance evaluations. Ensures adequate staffing coverage for theprovision of services on a day-to-day basis.

Motivates associates through staff appreciation, recognition, and incentive programs. Promotespositive associate relations and resolves grievances. Provides orientation, training, and education to staff on an ongoing basis.

This includes classroom, inservices and situation specific training. Ensures that community and corporate policies are disseminated and followed by all PersonalizedLiving associates. Assists in the development and implementation of the annual Personalized Living budget.

Works in conjunction with community associates and the administrator concerning the implementationand monitoring of program services and clinical program, if applicable and per state regulations. Ensures effective Performance Improvement Program. Partners with community administration in preparing for state survey or other regulatory processes.

Markets the Personalized Living program to any appropriate audience. Delivers and coordinates clinical services for the program, if applicable, and per state regulations. Responsible for the billing and scheduling of services or the delegation of such tasks.

Coordinates alternative resources for associates and families. Manages documentation process to ensure completeness, accuracy, timeliness and compliance withguidelines. Complies with all Safety and Infection Control, Universal Precautions, and OSHAstandards.

Maintains current knowledge of national and state requirements for scope of practice, supervision,credentialing and ongoing competency. Assumes responsibility for ongoing continuing education andprofessional development. Consistently demonstrates sound judgment in the evaluation, planning, implementation and follow-upof the Personalized Living programs.

Maintains positive relationships and rapport with coworkers, residents, family members andcommunity personnel. Participates in community committees as appropriate. Participates in handling "on call" duties after hours and weekends.

Responds to calls after hours and weekends in a timely manner. Available to staff working after normal business hours as a resource and manager.

Qualifications

Education and ExperienceLicensed Practical Nurse; or Registered Nurse, Bachelor's Degree in Nursing Registered Nurse, pursuant to stateregulations preferred. A minimum of five years related experience is required. Supervisory experience preferred.

Certifications, Licenses, and Other Special RequirementsMust be over the age of 18. Ability to work varying hours, including weekends and holidays.

Management/Decision MakingApplies existing guidelines and procedures to make varied decisions within the department. Uses sound judgmentand experience to solve moderately complex problems based on precedent, example, reasonableness or acombination of these.

Knowledge and SkillsPossesses extensive knowledge of a distinct skill or function and a thorough understanding of the organization andwork environment. Has working knowledge of a functional discipline.

Effective oral and written communicationskills. Excellent organizational and problem solving skills; strong supervisory skills; high degree of accuracy in allassignments; ability to work independently and manage time efficiently; high degree of initiative and creativity; goodjudgment; ability to employ professional ethics, and excellent analytical and clinical knowledge. Ability to project apositive and professional image at all times. Ability to manage multiple priorities and tasks.

Responsibilities Provides clients with assistance and companionship while recognizing clients' individual needs and encourages independence. Assists clients with activities of daily living, including bathing, dressing, grooming, toileting, transferring and getting to and from activities and meals according to the individual service plan. Allows and encourages clients to do as much of their own care as possible. Serves meals to clients in the dining room or apartments. May assist in preparing meals following preplanned menus. Encourages clients to eat a proper diet and take adequate fluids while respecting the client's right to free choice. Records and reports changes in client's eating habits to the supervisor. Initiates and participates in leisure activities provided for clients as described in the activity calendar. Encourages clients to socialize and participate in planned activities or programs and to develop friendships with other clients. Maintains clients' records daily in a timely manner and in accordance with company policy and procedures. This includes documentation related to: medication reminders as applicable; leisure activities; incidents and observations; reporting any changes in client's physical condition and/or behavior; visitors; etc. Observes and reports the health and emotional condition of each client, and promptly reports all changes to the supervisor. (Refer to state specific regulations regarding medication assistance and documentation). May assist clients with medication as defined in medication procedure. Assists or supervises clients who self-administer medication. If allowed by state regulations, follows physician orders and state laws to administer medications to clients who cannot self-administer their medications. Exhibits understanding of and follows medication policies and procedures. (Refer to state specific regulations regarding medication assistance). Maintains a clean, safe, and orderly environment for the clients. Performs general housekeeping following cleaning schedules for client laundry, bedrooms, dining area, living space, bathrooms, kitchen, etc. Follows proper procedures in emergency situations and responds promptly and positively to client requests for assistance - including emergency pull cords, telephone calls, and requests from family and friends. Knows location of each client, reminds them to sign out when they leave the building, and notifies the supervisor immediately if unable to locate a client.
