Position Summary:

As a key member of a multidisciplinary team, the Manager leads the evaluation of psychosocial and economic co-morbidities, actively participating in identification activities such as panel management. They engage in activities aimed at promoting holistic wellness. One of the Manager's primary responsibilities is the development and implementation of the CCP Concierge Care Coordination Social Worker Program. This involves designing strategies to enhance patient care coordination, establish effective linkages to community resources, and facilitate the achievement of care plan goals. In addition to their managerial duties, the Manager ensures that all social work staff perform their job duties and responsibilities in a courteous, customer-focused, and ethical manner. They uphold the highest standards of professionalism and ethical conduct while promoting a culture of compassionate care and patient advocacy within the department.

Essential Duties and Responsibilities:

  • Manages the daily operations within the Concierge Care Coordination Social Worker program.

  • Serves as a staff resource, responding to staff questions and assisting with problems.

  • Trains new staff on all aspects of the Social Worker Program, ensuring staff is up to date on software enhancements.

  • Conducts annual reviews of Population Health policies and procedures, ensuring compliance with contractual requirements.

  • Assists in creating new policies and procedures to meet AHCA, AAAHC, and other contract or accreditation requirements.

  • Promotes collaboration between Social workers and all other CCP departments for evidence-based care.

  • Monitors staff documentation for thorough psychosocial assessment, care planning, and evaluation.

  • Evaluates the development and monitoring of individualized care plans with the multi-disciplinary team.

  • Assesses population progress and outcomes to evaluate effectiveness of the program interventions and initiatives.

  • Mobilizes resources, intervenes as necessary, and evaluates initiatives to achieve expected goals.

  • Provides consultation to Population Health Care Managers for coordination with community resources.

  • Maintains familiarity with community resources and updates the community resources directory.

  • Educates staff on local and federal program requirements and limitations.

  • Maintains updated knowledge of managed care regulations, Medicaid/Social Security guidelines, and community agency programs.

  • Identifies and monitors potential quality concerns per corporate policy.

  • Collaborates with the population health team to monitor practice/process improvements and reports development/submission for effectiveness of workflow, service provision, and risk reduction.

Work Schedule:

As a continued effort to provide a safe and productive work environment, Community Care Plan is currently following a hybrid work schedule. Staff are able to work from home 3 days a week and will report to the office 2 days a week.

Qualifications

  • Minimum of a master's degree in Social Work or related field.

  • LCSW Licensure in State of Florida preferred.

  • Certified Case Manager (CCM) preferred.

  • Knowledge of Microsoft Word, Microsoft Outlook, Microsoft Excel, and internet software.
