The San Mateo Medical Center (SMMC) of the San Mateo County Health System is seeking a well-qualified individual for the position of Manager of Performance Based Delivery System Strategy (Program Services Manager II). The successful hire will have extensive experience working in a healthcare environment to perform a wide-variety of moderate to highly complex analytical and administrative support activities.
Under the general direction from the Quality Assurance Medical Director, the successful hire will work will work in collaboration with administrative, clinical, and business intelligence staff to support SMMC's delivery system transformation efforts.
This transformation work focuses on strengthening patient-centered care in the inpatient and outpatient settings, improving care coordination, and providing the right care in the most appropriate settings.
The ideal candidate will possess: Five years of Quality experience in a healthcare environment.
Three years of healthcare Quality Improvement-related project management.
Excellent written and verbal communication skills.
Proven skills and experience to formulate and present extensive narrative reports.
Excellent computer skills with knowledge of Microsoft Word, Excel, and a variety of electronic medical records(EMR)software (e.g.
Sorian,eClinicalWorks, and

PulseCheck). Ability to lead groups and projects to completion using Quality methodology.
Prior project planning experience, data analysis and data reporting.
This is a limited term, at-will position.
Limited Term Employees are assigned to assignments not to exceed three years.

Examples Of Duties Duties may include, but are not limited to, the following: Serve as a Project/Program Manager for SMMC's performance based system transformation efforts including, but not limited to, the Quality Incentive Program (QIP) and the Health Plan of San Mateo Pay for Performance Metrics.
Serve as a link between SMMC and the Outside Organization(s) sponsoring and providing technical assistance for the delivery system transformation work and mandated performance improvement.
Outside organization may include, but is not limited to, DHCS, Safety Net Institute, Health Plan of San Mateo, CMS.
Develop an in-depth understanding of each Project's associated metric specifications as outlined by the sponsoring Outside Organization.
Develop an in-depth understanding of the performance improvement necessary for each metric within the project and communicate this knowledge to key SMMC stakeholders.
In accordance with the Outside Organization's reporting guidelines, develop, validate and submit the mandatory reports for SMMC's transformation efforts and associated performance metrics.
Work with Population Health Management analytics, the Health System's Business Intelligence Team, and SMMC Clinical leadership to develop and implement solutions to obtain the data required to complete required performance metric reporting.
Work with the Business Intelligence Project Manager to develop a mutually agreed upon timeline and strategy for obtaining and reporting the data required to submit the Outside Organization's mandatory performance improvement reports.
Understand the clinical strategy for the performance improvement work required for each performance metric.
Utilize this understanding to work with Clinical Sponsors, eCW, and MITS team members and individual clinicians to make changes in clinical documentation so that accurate data is captured.
Work with Business Intelligence team members, clinicians, and other partners to explore data questions, validate data, and maintain quality of the data.
Coordinate communication between clinicians, BI, Soarian, eCW, Pulsecheck teams.
As needed, support local clinical teams and the LEAP office in development of interventions necessary to achieve the required system transformation and improve health outcomes for patients.
Support this work by utilizing knowledge of evidence-based quality improvement methods.
Work with the Business Intelligence team and Clinical Sponsors to track each project's overall progress towards meeting the necessary metric performance improvements.
Identify within each project, the unique risks and areas for opportunity for improvement and communicate these finds in a timely manner to each improvement team and Clinical Sponsor.
Upon request, participate in discussions with finance and executive teams to prioritize the improvement efforts related to each project.
Perform related duties as assigned.
Qualifications Knowledge of:
Principles and practices of public administration, and program management, including planning, implementation and evaluation.
Principles and methods of community and social services including current trends in education, research, training, treatment, intervention, rehabilitation, or related issues.
Laws, codes and regulations governing social and community service programs, and knowledge of County, state and federal legislative developments applicable to contemporary program or service issues.
Organizational and fiscal management within a multi-faceted, community-based program setting.
Community resources and public/private services and funding sources.
Skill/Ability to:
Identify and analyze complex socioeconomic issues and problems.
Plan, organize, and coordinate programs and services on a comprehensive, county-wide level to meet community needs.
Integrate a variety of activities and services to achieve program goals, objectives, and priorities.
Apply principles and techniques of community organization.
Speak effectively to diverse audiences, including client, professional, civic and citizen groups.
Maintain cooperative working relationships with other program managers and staff, other agencies and the community; enlist and mobilize community support for programs.
Prepare complex and detailed written reports, program policies, procedures and contracts.
Train staff and coordinate activities of contractors, volunteer groups and staff.
Education and Experience:
Any combination of education and experience that would likely provide the required knowledge, skills and abilities is qualifying.
A typical way to qualify is: Three years of experience in a health, social or community services organization, including at least two years of significant responsibility for one or more of the following: program and budget development, funding, utilization, coordination of community resources or program evaluation.
