  • Essential Duties and Responsibilities include the following:

1.Oversees day-to-day activities, care coordination and provision of Outpatient Case Managers.

2.Has direct oversight responsibility and accountability of the program and department of case management and ancillary staff in RMG North.

3.Manages and monitors enrollees assigned to Outpatient Case Management programs to ensure efficient and timely delivery of care, timely referrals, and moving the enrollee's treatment plans through the care continuum.

4.Develop a standardized process for triaging and enrollment of appropriate members into Outpatient Case Management programs for RMG North.

5.Identifies and develops program and department policies and procedures and ensures compliance and consistency throughout RMG regional sites.

6.Develops and coordinates program and department strategies for ensuring the delivery of care in the most cost effective setting.

7.Develops, monitors, and analyzes program and department reports and identifies trends to improve clinical and utilization and quality outcomes.

8.Develops annual program and department goals and objectives focused on improving patient outcomes and efficiency.

9.Implements tools and processes that direct and role model a high level of customer service behavior toward enrollees.



  1. Works collaboratively with the entire Medical Management team to ensure that Employer company goals as well as compliance with regulatory and health plan requirements are met.

  2. Act as a resource for hospitalists, SNFs, outpatient utilization management staff, home health care agencies, Employer home-visit physicians and nurse practitioners.

  3. Training, mentoring, auditing of the clinical competency, productivity, contribution, customer service interaction of Program and Department professional and non professional staff. Act as a resource to all staff members of the Program and Department.

  4. Motivates, instructs, mentors, audits and leads and constantly improves the quality of service and clinical outcomes rendered by the Employer's Outpatient Case Management staff.

  5. Provide regular audit reports on Program and Department staff and action plans to improve efficiency and efficacy.

  6. Manager shall be based in the Northridge site. Manager's service area jurisdiction and responsibilities may be expanded and/or changed at Employer's discretion.

  7. All other duties as directed by management.


  • Education and/or Experience:

1.Graduate from an accredited Registered Nursing Program.

2.Current CA RN license, current CPR certification, valid CA Driver's license.

3.4-5 years of previous management experience required.

4.3 years acute care or case management experience preferred.

5.2-3 years of HMO experience preferred.

6.Must have experience in managing high risk cases.

7.High level knowledge of Case Management processes and principles

8.Prior experience in MSO environment preferred.

9.Ability to work with all levels of management.



  1. Must have excellent verbal and written communications skills.

  2. Must have excellent organizational skills.

  3. Must have prior experience with project development and implementation.

  4. Must have working knowledge of MS Office environment

  5. Requires current CA driver's license and car insurance

The Case Manager Outpatient RN is responsible for the assessment, treatment planning, intervention, monitoring, evaluation and documentation on identified High Risk members. The Case Manager Outpatient RN will assess and develop a care plan in collaboration with the admitting, attending and consulting physician, the member and other health care practitioners. The goal of the Case Manager Outpatient RN is to effectively manage members on an outpatient basis to assure the appropriate level-of-care is provided, to prevent in patient admission and re-admissions, and ensure that the members' medical, environmental, and psychosocial needs are met over the continuum of care. Essential Duties and Responsibilities include the following: 1. Keeps member/family members or other customers informed and requests if necessary, further assistance when needed. 2. Demonstrates the ability to follow through with requests, sharing of critical information, and getting back to individuals in a timely manner. 3. Functions as liaison between administration, members, physicians and other healthcare providers. 4. Interacts professionally with member/family/physicians and involves member/family/physicians in formation of the plan of care. 5. Performs a Clinical Assessment/Questionnaire of the member and determines an acuity score for necessary scheduled follow-up. 6. Develops an outcome-based plan of care, based on the member's input and assessed member needs. Implements and evaluates the plan of care as often as needed as evidenced by documentation in the member's case file. 7. Documents member assessment and reassessment, member care plans, and other pertinent information completed in the member's medical record in accordance with the FOCUS Charting methodology, nursing standards, and company policies and procedures. 8. Initiates community visits (hospital, home visits) as needed to assess patient progress and meet with appropriate members of the patient care team. 9. Identifies planned and unplanned transitions of care from Requests for Services or daily inpatient and SNF census. 10. Educates the member/caregiver on the transition process and how to reduce unplanned transitions of care. 11. Manages transition of care from the sending to receiving settings ensuring that the Plan of Care moves with the member and updates/modifies the care plan as the member's health care status changes. 12. Communicates appropriately and clearly with physicians, in patient case managers and Prior-Authorization nurses 13. Identifies and addresses psychosocial needs of the members and family and facilitates consultations with Social Worker, as necessary. 14. Identifies and addresses pharmacological needs of the members and facilitates consultations with the pharmacy department, as necessary. 15. Identifies community resources to address needs not covered by the member's benefit plan, and coordinates member benefits as needed, with the health plan. 16. Participates in the efficient, effective and responsible use of resources such as medical supplies and equipment. 17. Responsible for the coordination and facilitation of member and family conferences as determined by assessment of member's needs. 18. Identifies the appropriate members to participate in the interdisciplinary case round process. Prepares the necessary summary information to present to the team. 19. Responsible for the coordination of clinic appointments, medication reconciliation, PCP and SPC visits. 20. Ability to collaborate and communicate with all members of the healthcare team (concurrent review, pre-authorization, PCP/SPC, Social Services, and Pharmacy) to coordinate the continuum of care of developing plans for management of each case. 21. Responsible for the identifying members that are appropriate for hospice conversion or Palliative care. 22. Meet with members/caregiver face to face in different locations (clinic, home, hospital, and community) in order to build a rapport with member so that the case manager can better support member/caregiver with care coordination and the plan of care. 23. Other duties as assigned by management. Education and/or Experience: 1. Graduate from an accredited Registered Nursing Program. 2. Must have valid CA DL and provide proof of vehicle insurance. 6. Knowledge of computers, faxes, printers and all other office equipment. 7. Typing 30 WPM with accuracy. 8.
