Position Summary:

Under the guidance and supervision of the Director of Case Management (CM), the CM Manager is responsible and accountable for coordination of services through an interdisciplinary process that provides a clinical, financial, and psychosocial approach through the continuum of care.

  • Balances individually identified patient and family needs with the efficacious and cost effective utilization of resources.

  • The CM Manager is responsible for the evaluation, planning, implementation, coordination and supervision of Case Management Department operations, functions and staff in alignment with the UM/CM program as well as applicable regulations and standards.

  • Responsible for supervising day to day operations, including the staff schedule, assignments and caseloads to ensure appropriate hospital wide coverage.

  • Provides expertise and staff support for problem solving complex cases and issues and consults with the Director/other resources as needed.

  • Collaborates with Director and participates in staff evaluation and performance activities including monitoring, coaching, counseling, educating and developing action plans as needed.

  • Represents CM and attends meeting for assigned committees, workgroups and teams. Participates in required CM department data collection, medical record reviews/audits, reporting and analysis.

  • Develops and supervises new staff orientation plans and the department's annual education plan including staff core competencies.

  • Shares responsibility with the Director for some "24/7" management accountability and provides coverage for the Director's time off as needed.

  • Demonstrates an understanding of the CM department's budget and practices cost-effective resource utilization.

RN; BSN required or MSW; LCSW required


Five or more years clinical experience required, hospital setting preferred; Three year UM/CM experience required; One to three years charge/lead/supervisory/management experience preferred; Able to demonstrate leadership and management skills

Special Qualifications:

Current CO licensure or able to get CO licensure within first month of employment; current CPR certification or be certified within first month of employment; UR/CM certification preferred

Degree of Supervision Required:

Involves general guidance and direction by the Director. Is expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures.

Description of Job As a part of the Department of Health Care Policy and Financing's Executive Leadership Team, the Case Management & Quality Performance (CMQP) Division Director is responsible for the oversight and administration of Colorado's long-term services and supports case management system. This position will be responsible for working across the Department to achieve this goal. This position is responsible for leading efforts to redesign Colorado's case management system and for the operation and oversight of the system as it exists today. For all of the programs within the Division, the Division Director is responsible for ensuring: * Operational excellence * Sound financial management * Excellent member and stakeholder relationships and responsiveness * Quality implementation of new programs and program enhancements in an expeditious manner * Quality staffing This position manages the daily operations of multiple work groups through subordinate professional program supervisors. These sections include: * Quality Performance Section * Case Management Section * Entry Point Evaluation & Development Section In these areas of programmatic authority, position is responsible for implementing long-range strategic plans to support the Department's overall mission and objectives. Position must negotiate with other managers having strategic planning authority to ensure consistency, care delivery efficiency, and integration in implementation.
